Abstract

U neurodegenerativnim demencijama kao što je Alzheimerova bolest oštećenje kognitivnih funkcija dominantan je poremećaj koji bitno utječe na bolesnikov svakodnevni život. Također, može negativno utjecati na bolesnikovo samopouzdanje i dovesti do povlačenja iz svakodnevnih aktivnosti. Intervencije kognitivne rehabilitacije važne su za bolesnika s demencijom jer im omogućuju veću neovisnost i poboljšavaju njihovo kognitivno funkcioniranje. Kognitivna rehabilitacija nije usmjerena samo na poboljšanje specifičnih kognitivnih funkcija, već na sve komponente kognitivnog deficita koje promatra u kontekstu
ponašanja i socijalnih funkcija. Ovaj model rehabilitacije obuhvaća kognitivnu stimulaciju, kognitivni trening i specifične intervencije kao što su "reality orientation therapy", "reminiscence therapy", "errorless learning" i "process-based" kognitivni trening. Budućnost kognitivne rehabilitacije Alzheimerove bolesti leži u daljnjem razvijanju intervencija koje su umjerene na specifične probleme, ali i na sveukupno kognitivno funkcioniranje bolesnika s demencijom. Također, važno je i razvijanje pristupa rehabilitacije koji imaju utjecaj na pridržavanje treninga i njegovu provedbu.

Abstract (english)

The dominant disorder in neurodegenerative dementias, such as Alzheimer’s disease, is cognitive impairment. It affects patient’s everyday life and it can negatively impact on his or hers self-confidence and it can lead to a withdrawal from normal everyday activities. Interventions of cognitive rehabilitation are an important aspect of care for patients with dementia because they provide with more independence and improves their cognitive functioning. Cognitive rehabilitation is not only directed towards the improvement of specific cognitive functions, but it also includes all components of cognitive deficit which are considered in the context of behavior and social functioning. This model of treatment encompasses cognitive stimulation, cognitive training and specific interventions such as reality orientation therapy, reminiscence therapy, errorless learning and process-based cognitive training. The future of cognitive rehabilitation in patients with Alzheimer’s disease is in the further development of interventions directed at specific problems, but total cognitive functioning is also considered important. It is important to instruct patients to participate in trainings which help them to comply with those programs.
