Abstract

Zatajivanje srca definira se kao klinički sindrom karakteriziran tipičnim simptomima (zaduha, slabost, umor) i fizikalnim nalazom (bilateralnim edemom potkoljenica, auskultatorno plućnim krepitacijama, povećanim jugularnim venskim tlakom), a koji se javljaju kao posljedica strukturnog i/ili funkcionalnog poremećaja srca koji za posljedicu ima smanjenu istisnu frakciju i/ili povećane tlakove punjenja lijevog ventrikula tijekom odmora ili tjelesnog napora. Procjenjuje se da od zatajivanja srca boluje 1-2% odrasle populacije u razvijenim zemljama uz porast na više od 10% u osoba starijih od 70 godina. Najčešći uzroci zatajivanja srca su ishemijska bolest srca, idiopatska dilatativna kardiomiopatija, hipertenzija i bolest srčanih zalistaka. Prisutnost komorbiditeta u oboljelih sa zatajivanjem srca je od velike važnosti obzirom da otežavaju dijagnostički i terapijski proces, pogoršavaju simptome i kvalitetu života te doprinose višim stopama hospitalizacije i mortaliteta. Aktualne smjernice Europskog kardiološkog društva ukazuju na to mijenja li prisutnost zatajivanja srca u ovih bolesnika način na koji se komorbiditeti inače liječe, obzirom da liječenje može pogoršati osnovnu bolest. Stoga pravovremeno prepoznavanje i pravilno liječenje komorbiditeta ima ključnu ulogu u sveobuhvatnom pristupu ovim bolesnicima.

Abstract (english)

Heart failure is a clinical syndrome characterized by typical symptoms (breathlessness, weakness, fatigue), that may be accompanied by signs (bilateral leg edema, pulmonary crackles, elevated jugular venous pressure), caused by a structural and/or functional cardiac abnormality, resulting in a reduced cardiac output and/or elevated intracardiac pressures at rest or during stress. Approximately 1-2% of the adult population in developed countries suffers from this condition, rising to more than 10% among people over 70 years of age. The most common causes of heart failure include the following: ischemic heart disease, idiopathic dilated cardiomyopathy, hypertension and valvular heart disease. Comorbidities are of great importance in heart failure, since they interfere with the diagnostic and therapeutic process, aggravate symptoms and impair quality of life, contributing to higher hospitalization and mortality rates. The current European Society of Cardiology (ESC) guidelines suggest if the presence of heart failure should change the way a comorbidity would normally be treated, because treating may aggravate the underlying disease. Therefore, timely diagnosis and proper treatment play a crucial role in a comprehensive approach to these patients.
