Abstract

Cushingov sindrom (CS) je poremećaj koji nastaje kao posljedica kronične izloženosti prekomjernim razinama endogenih ili egzogenih glukokortikoida. Endogeni Cushingov sindrom s obzirom na etiologiju možemo podijeliti na ACTH-ovisni i ACTH-neovisni CS. Najčešći uzrok ACTH-ovisnog CS je kortikotropni adenom hipofize (Cushingova bolest), dok je najčešći uzrok ACTH-neovisnog oblika CS adenom ili karcinom nadbubrežne žlijezde. Cilj ovog istraživanja bio je usporediti parametre metaboličkog sindroma (hipertenzija, diabetes mellitus, hiperlipidemija) i učestalost korištenja statina između pacijenata s ACTH-ovisnim i ACTH-neovisnim CS. Analizirani su parametri 53 pacijenta s endogenim CS (45 žena i 8 muškaraca raspona dobi 23-73 godine, prosječne dobi 44,3 ± 14,6 godine), liječenih na Zavodu za endokrinologiju Klinike za unutarnje bolesti Kliničkog bolničkog centra Zagreb u razdoblju od svibnja 2011. do ožujka 2018. 32 pacijenta je imalo dijagnozu ACTH-ovisnog CS, a ostali pacijenti (21) su imali ACTH-neovisni oblik CS. Rezultati su pokazali veću učestalost hipertenzije u pacijenata ACTH-neovisnim CS (90%) u odnosu na pacijente s ACTH-ovisnim oblikom CS (71%) iako navedena razlika nije bila statistički značajna. Također nije bilo statistički značajne razlike u učestalosti DM, hiperlipidemije i korištenja statina između obje skupine pacijenata. Statističkom analizom parametara (DM i hipertenzija) između muškaraca i žena neovisno o etiologiji CS se isto tako nije mogla naći nikakva značajna razlika, iako brojne studije pokazuju češće komplikacije kod muškaraca pa tako naše rezultate možemo pripisati manjem broju ispitanika.

Abstract (english)

Cushing's syndrome (CS) is a disorder that is a result of chronic exposure to excessive levels of endogenous or exogenous glucocorticoids. Endogenous Cushing's syndrome by etiology can be divided into ACTH-dependent CS and ACTH-independent CS. The most common cause of ACTH-dependent CS is corticotropic pituitary adenoma (Cushing’s disease), while the most common cause of AC-independent form of CS is an adenoma or adrenal carcinoma. The aim of this study was to compare the parameters of the metabolic syndrome (hypertension, diabetes mellitus, hyperlipidemia) and the frequency of statin use between patients with ACTH- and ACTH-dependent CS. The parameters of 53 patients with endogenous CS (45 women and 8 men aged 23-73 years, mean age 44.3 ± 14.6 years), treated at the Department of
Endocrinology of the Clinic for Internal Medicine of the Clinical Hospital Center Zagreb in the period from May 2011 to March 2018, were analyzed. 32 patients were diagnosed with ACTH-dependent CS, and the remaining patients (21) had an ACTH-independent form of CS. The results showed a higher incidence of hypertension in patients with ACTH-independent CS (90%) compared with patients with ACTH-dependent CS (71%), although this difference was not statistically significant. There were also no statistically significant differences in the incidence of DM, hyperlipidemia, and the use of statins between patient groups. Statistical analysis of parameters (DM and hypertension) between men and women regardless of the etiology of CS also found no significant difference, although numerous studies show more frequent complications in men, so our results can be attributed to a smaller number of subjects.
