Abstract
Ciljevi: Primarni cilj rada bio je ustanoviti je li provedba inicijative „Rodilište - prijatelj djece“ dovela do promjene načina prehrane novorođenčadi kod otpusta iz bolnice. Sekundarni cilj ovog stručnog rada bio je ustanoviti u kolikoj mjeri Inicijativa „Rodilište - prijatelj djece“ promjenom načina prehrane novorođenčadi, ukoliko se ista potvrdi, doprinosi promijeni parametara koji su povezani s dojenjem, a mogu potencijalno utjecati na perinatalni i rani neonatalni morbiditet. Ispitanici i metode: Podatci su prikupljeni retrospektivno iz povijesti bolesti novorođenčadi rođenih 1993. i 2015. godine u KB Merkur. Kriterijima uključenja i isključenja definirana je skupina zdrave novorođenčadi rođenih iz trudnoća majki, koje su rađale prvi puta vaginalnim putem u stavu glavom bez zabilježenih komplikacija tijekom porođaja ili pridružene patologije trudnoće. Provedena je deskriptivna analiza podataka te je korišten Mann Whitney test. Za razinu statističke značajnosti odabrano je p<0,05. Rezultati: od ukupno 2558 rođenih u 1993. godini i 1526 rođenih u 2015., u analizu je uključeno 105 novorođenčadi iz 1993. i 99 novorođenčadi iz 2015. Prosječna tjelesna težina u 1993. godini iznosila je 3.517 grama, a tijekom 2015. godine iznosila je 3.318 grama. Tjelesna težina pri otpustu novorođenčadi u 1993. godini iznosila je 3.401 grama, a 2015. godine 3.235 grama. U načinu hranjenja novorođenčadi 1993. prevladava kombinirana prehrana, dok se u 2015. godini ishrana bazira na isključivom dojenju. Utvrđena je statistički značajna razlika u načinu prehrane novorođenčadi od 1993. do 2015. godine (z=13,02, p<0,001). Godine 2015. rodilje i novorođenčad nakon porođaja u rodilištu borave kraće nego 1993. godine (t=6.15, p<0.0001), te je vrijeme od porođaja do prvog podoja također bilo kraće 2015. u usporedbi s 1993. godinom (t=11.68, p<0,0001). Zaključak: Porast broja dojene djece kod otpusta iz bolnice rezultat je provedbe inicijative „Rodilište - prijatelj djece“. Također nije utjecao na pad u tjelesnoj težini, te je smanjio duljnu boravka u bolnici kao i vrijeme od porođaja do prvog podoja.

Aims: The primary aim of this study was to determine if implementation of «Baby-friendly hospital» initiative has changed the mode of feeding among newborns at hospital discharge. Secundary aim of this study was to establish in what way does change in mode of feeding, if it is determined as such, contribute to change of parameters related to breastfeeding which might potentially influence perinatal and early neonatal morbidity. Examinees and methods: The study was performed using data from medical records of newborns born in years 1993 and 2015 at University Hospital Merkur. It compared 105 newborns from 1993 and 99 newborns form 2015 born vaginally in occiput anterior preesentation from normal low risk pregnancies from nuliparous mothers wihout observed labour complications or patologies during pregnancies. Both groups were selected according to same exclusion and inclusion criteria. Statistical analysis was performed using Mann Whitney test. Statistical signficance was set to p<0.05. Results: Among a total of 2558 newborns in year 1993, and 1626 in 2015, 105 newborns from year 1993 and 99 from year 2015 were found to fulfill selected exclusion and inclusion criteria. Mean brithweight in 1993 was 3517 grams and 3316 grams in 2015, while discharge newborn weight was 3401 grams in 1993 and 3235 grams in 2015. A statistically significant difference was observed between 1993 and 2015 regarding mode of feeding; in 1993 the majority of newborns were fed using combined feeding, while in 2015 the prevailig mode was breastfeeding (z=13,02, p<0,001). Women and newborns stayed less in hospital in 2015 (t=6.15, p<0.001) and the time between birth and first feeding was shorter in 2015 (t=11.68, p<0,001). Conclusion: This rise of the number of newborn fed by breastfeeding at hospital discharge is a result of implementation of „Baby-friendly hospital“ initiative. Also, this rise the number of breastfeed newborns did not lead to a decrease in newborn in newborn wight, but it has shortened the time newbons spent in hospital after birth and the time between birth and first feeding.
