Abstract

Cilj: Cilj rada je usporediti patohistološki odgovor na neoadjuvantnu terapiju u dvije skupine pacijentica s ranim rakom dojke, HER2 negativne luminalne u odnosu na HER2 pozitivne, luminalne i neluminalne.
Metode: U istraživanje je uključeno 42 pacijentice s različitim podtipovima karcinoma dojke: luminalni HER2 negativan te HER2 pozitivan, uključujući neluminalni i luminalni podtip. Kod svih pacijentica je neoadjuvantna terapija (NAT) u potpunosti provedena. Statistička analiza podataka provedena je Shapiro-Wilkovim testom.
Rezultati: Rezultati su pokazali da prije provedene NAT u skupini od 22 pacijentice s HER2 pozitivnim karcinomom dojke, 8 je bilo u stadiju 2a, 9 u stadiju 2b, a 3 u stadiju 3a. Nakon provedene NAT, sve pacijentice u ovoj skupini su se našle u stadijima 0 i 1a, od čega je gotovo dvije trećine (N=13; 59,09%) postiglo stadij 0. Sve HER2 pozitivne pacijentice na NAT su odgovorile s RCB-0 (potpunim patološkim odgovorom) ili RCB-I. Statistički značajno smanjenje površine tumora zabilježeno je u obje ispitivane skupine nakon provedene terapije.
Zaključak: Pacijentice s HER2 pozitivnim rakom dojke pokazale su bolji odgovor na neoadjuvantnu terapiju u usporedbi s pacijenticama s HER2 negativnim rakom dojke te je biološka terapija dualnom HER2 blokadom; trastuzumabom i pertuzumabom rezultirala značajno većom stopom potpunog patološkog odgovora.

Abstract (english)

Objective: The aim of this study is to compare the response to neoadjuvant therapy in a group of patients with HER2 negative luminal breast cancer with a group of patients with HER2 positive breast cancer, including luminal and non luminal.
Methods: The study included 42 patients with different breast cancer subtypes: luminal B HER2 negative breast cancer and HER2-positive breast cancer, including both non-luminal and luminal B subtypes, who completed neoadjuvant therapy (NAT). Statistical data analysis was performed using the Shapiro-Wilk test.
Results: The results showed that prior to NAT, out of the 22 patients in the HER2-positive group, 8 were in stage 2a, 9 in stage 2b, and 3 in stage 3a. After NAT, all patients in this group were in stage 0 or 1a, with almost two-thirds (N=13; 59.09%) achieving stage 0. All HER2-positive patients responded to NAT with either RCB-0 (pathological complete response) or RCB-I. A statistically significant reduction in tumor size was observed in both examined groups after therapy.
Conclusion: Patients with HER2 positive breast cancer showed a better response to neoadjuvant therapy compared to HER2 negative patients. Targeted therapy with dual HER2 blockade, using trastuzumab and pertuzumab, resulted in significantly improved outcomes, as evidenced by a pathological complete response.
