Abstract
Palijativna skrb je aktivna sveobuhvatna skrb za bolesnika čija bolest ne reagira na postupke aktivnog liječenja. U palijativnoj skrbi je kvaliteta života ispred duljine života. Da bi se to postiglo potrebno je objedinjenje fizičkih, socijalnih, psiholoških i duhovnih oblika pomoći. Velika većina palijativnih bolesnika želi umrijeti u svom domu. Bolnice su najskuplje mjesto za pružanje palijativne skrbi dok kućna palijativna skrb ima mnoge prednosti za palijativnog bolesnika i njegovu obitelj. Cjelovita skrb te koordinacija zdravstvenog zbrinjavanja pojedinca, obitelji i lokalne zajednice zahtjeva od liječnika obiteljske medicine timski rad i multidisciplinarni pristup pri zbrinjavanju populacije koja ga je izabrala. Ako je potrebno tim se proširuje uključivanjem zdravstvenih i nezdravstvenih stručnjaka. Mobilni specijalistički tim palijativne skrbi koji djeluje u Istarskoj županiji u bitnome pridonosi pružanju multidisciplinarne, timske i sveobuhvatne palijativne skrbi. Kvalitetna palijativna skrb na primarnoj razini je nužna jer unapređuje kvalitetu bolesnikova života i cjelokupne zdravstvene zaštite.

Palliative care is the active comprehensive care of patients whose disease does not respond to actions of active treatment. In palliative care, the quality of life matters more than its length. In order to achieve this it is necessary to the unification of physical, social, psychological and spiritual forms of assistance. The large majority of palliative patients want to die at home. Hospitals are the most expensive place to provide palliative care while home palliative care has many benefits for palliative patients and their families. Complete care and coordination of health care for individuals, families and local communities demands from family physicians to use teamwork and multidisciplinary approach to the care of the population who elected him. If necessary, this expands the inclusion of health and non-health professionals. Mobile specialist palliative care team that works in the County of Istria substantially contributes to the provision of multi-disciplinary, team and comprehensive palliative care. High-quality palliative care at the primary level is necessary because improving the quality of life of the patient and the entire health care system.
