Abstract

Učestalost nasljednih trombofilija u općoj populaciji iznosi 10 % - 15 %. Utjecaj nasljednih trombofilija na trudnoću intenzivno se istražuje unatrag dvadesetak godina. Prema rezultatima radova najviše snage dokaza i smjernicama vodećih svjetskih ginekoloških i hematoloških društava nasljedne trombofilije ne utječu na komplikacije u trudnoći kao što su habitualni pobačaji, preeklampsija, abrupcija posteljice, fetalna smrt i fetalni zastoj rasta. Navedene komplikacije u trudnoći nisu indikacija za testiranje na nasljedne trombofilije.
S druge strane one su rizični čimbenik za tromboembolijsku bolest i uzrokuju preko 50 % tromboembolija u trudnoći i babinju. Indikacija za profilaksu ovisi o tipu nasljednih trombofilija i osobnoj i obiteljskoj anamnezi. Prema ovim smjernicama profilaksa tromboembolije je apsolutno indicirana kod visokorizičnih trombofilija homozigot za FVL i protrombin G20210A i kod dvostrukih heterozigota za ove čimbenike. Postoje razlike među smjernicama glede profilakse u slučajevima antitrombin deficijencije, protein C i protein S deficijencije. Prema nekim smjernicama kod ovih deficijencija profilaksa ovisi o anamnestičkim podacima dok je kod drugih smjernica za profilaksu dostatan podatak o deficijenciji navedenih čimbenika. Sve smjernice naglašavaju važnost profilakse u babinju. MTHFR, PAI 1, homocistein nemaju nikakvog utjecaja na komplikacije trudnoće niti tromboemboliju te su testiranja na ove čimbenike nepotrebna.

Abstract (english)

The incidence of inherited thrombophilias in the general population is 10% - 15%. The influence of inherited thrombophilias on pregnancy has been intensively researched for twenty years. According to the results of the research with the highest avaliable evidence and the guidelines of the world's leading gynecological and hematological societies, inherited thrombophilias do not affect pregnancy complications such as habitual miscarriages, preeclampsia, placental abruption, fetal death and fetal growth restriction. These complications in pregnancy are not an indication for testing for inherited thrombophilia.
On the other hand, they are a risk factor for thromboembolic disease and cause over 50% of thromboembolisms in pregnancy and puerperium. The indication for prophylaxis depends on the type of thrombophilia and personal and family history. According to all the cited guidelines, thromboembolism prophylaxis is indicated in high-risk thrombophilias homozygous for FVL, homozygous for prothrombin G20210A and in double (compound) heterozygotes. There are differences among the guidelines regarding prophylaxis in cases of antithrombin deficiency, protein C and protein S deficiency. All guidelines emphasize the importance of prophylaxis in the puerperium. MTHFR, PAI 1, homocysteine do not influence on pregnancy complications or thromboembolism, so testing for these factors is unnecessary.
