Abstract

Kronični svrbež (KS) je vodeći simptom u mnogim dermatološkim i sistemskim bolestima i prisutan je u oko 8,4-22,6% opće populacije. Kronični svrbež, jednako kao i kronična bol, umanjuje kvalitetu života, ometa pacijenta u svakodnevnim aktivnostima i utječe na opće stanje pacijenta. Patofiziološki mehanizmi kroničnog svrbeža nisu još uvijek u potpunosti razjašnjeni.
U oko 10-50% odraslih, svrbež može biti važan simptom mnogih sistemskih bolesti, poput bubrežne insuficijencije, kolestaze, hematoloških poremećaja, endokrinoloških ili malignih bolesti.
Pacijentima s KS potreban je individualan pristup i liječenje koje odgovara stanju kože (praćeno upalom kože ili bez nje), mogućoj sistemskoj bolesti koja se nalazi u podlozi ovog simptoma te postojećoj primjeni lijekova.
Terapija svrbeža može biti lokalna ili sistemska. Antihistaminici su prvi izbor kod KS, dok se fototerapija preporuča za generalizirani svrbež, antikonvulzivi te analgetici kod neuropatskog svrbeža, a antidepresivi kod KS koji je rezistentan na terapiju. Sistemski glukokortikosteroidi se ne preporučuju kao terapija svrbeža, osim u iznimnim i teškim oblicima.

Abstract (english)

Chronic pruritus (CP) is a major symptom in numerous dermatological and systemic diseases affecting 8,4-22,6% of population. CP can have a dramatic impact on the quality of life, similar to chronic pain, and it can worsen the general condition of the patient. The pathophysiological mechanisms are still insufficiently understood.
In 10–50% of adults with persistent pruritus, it can be an important dermatologic clue for the presence of a significant underlying systemic disease such as renal insufficiency, cholestasis, hematologic disorder, endocrine disorder or malignancy.
Patients with chronic pruritus require an individually therapy based on the condition of the skin (inflamed or non-inflamed), possible underlying causes and existing co-medication.
Treatment for pruritus can be local or systemic. Antihistamines are fist-choice therapy in CP, while phototherapy is recommended for generalized pruritus, anticonvulsants and analgetics in neuropathic pruritus, and antidepressants in forms of CP not responding to other therapies. Systemic glucocorticosteroides are not recommended for treatment of CP except for severe cases.
