Abstract

Fiziološke promjene u funkciji štitnjače za vrijeme trudnoće uzrokovane su hormonskim promjenama i metaboličkim zahtjevima, specifičnim za trudnoću, koje dovode do promijenjenih serumskih koncentracija hormona štitnjače i tireotropina. Varijabla koja se određuje u probiru na poremećaje funkcije štitnjače je serumska koncentracija tireotropina. Pri interpretaciji testova funkcije štitnjače nužni su za tromjesečje specifični referentni rasponi kako bi se primijenilo adekvatno hormonsko i tireostatsko liječenje. Normalne vrijednosti koncentracije tireotropina su modificirane tako da je gornja granica referentnog raspona 2.5 mUI/l u prvom tromjesečju, a 3.0 mUI/l u drugom i trećem tromjesečju trudnoće. Autoimunosna bolest štitnjače je najčešći organski specifičan autoimunosni poremećaj, učestao u žena u reproduktivnoj dobi, koji obično dovede do disfunkcije štitnjače. Klinički manifestna kao i subklinička disfunkcija štitnjače povezane su s povišenim rizikom komplikacija za vrijeme trudnoće. Hormoni štitnjače potrebni su za metaboličke procese u ljudskom tijelu kao i za normalan razvoj fetusa, osobito fetalnog središnjeg živčanog sustava. Trudnoća utječe na tijek bolesti štitnjače, a autoimunosne bolesti štitnjače mogu utjecati na tijek i ishod trudnoće. Žene s poremećenom funkcijom štitnjače koje planiraju trudnoću, kao i one s povećanim rizikom razvoja poremećaja funkcije štitnjače treba uključiti u probir određivanjem koncentracije serumskog tireotropina prije trudnoće ili u ranoj trudnoći i ukoliko je potrebno započeti s liječenjem.

Abstract (english)

Physiological changes in thyroid function during pregnancy are affected by hormonal changes and metabolic demands that occur during pregnancy, resulting in modification of thyroid hormones and thyrotropin serum concentrations. Thyrotropin serum concentration is a screening variable for thyroid dysfunction. Trimester-specific reference ranges for thyroid function tests are substantial for adequate thyroid hormone supplementation and antithyroid drug therapy throughout pregnancy. Normal values for serum thyrotropin have been modified recently, the upper normal range is now considered to be 2.5 mUI/l in the first trimester and 3.0 mUI/l in second and third trimester. Autoimmune thyroid disease is the most common organ specific autoimmune disorder prevalent in women of childbearing age, usually resulting in dysfunction of the thyroid gland. Both overt and subclinical thyroid dysfunction are related with increased risk of complications during pregnancy. Thyroid hormones are necessary for metabolic processes in human body as well as for normal development of fetus, especially their central nervous system. Pregnancy may affect the course of these thyroid disorders, and conversely, autoimmune thyroid diseases may affect the course of pregnancy. All women considering pregnancy with known thyroid dysfunction, and those women who are at high risk for developing thyroid dysfunction, should be tested for abnormal thyrotropin concentration before pregnancy or in early pregnancy, and if necessary properly treated.
