Abstract

Dan danas, za većinu trudnica, porod predstavlja jedno bolno iskustvo koje se ponekad uspoređuje s kompleksnim bolnim sindromima ili čak amputacijom prsta. Unatoč činjenici da bol u porodu nije životno ugrožavajuća za većinu zdravih trudnica, ipak može uzrokovati anksiozne posljedice. Pokazalo se da je veća učestalost postnatalne depresije kad nije primijenjena analgezija u porodu, te je bol tokom poroda u korelaciji s razvojem post-traumatskog stresnog poremećaja.
Do sada se pokušavalo s raznim načinima, kako farmakološkim (sistemski opiodi počeli su se koristiti 1840.-te godine te su do danas postali najčešće korišteni lijekovi) tako i nefarmakološkim (TENS, masaža, vodena kupka, akupunktura, hipnoza) ublažiti bol. Međutim, na porođaj se treba gledati kao na jedno multifaktorsko iskustvo kad u obzir uzmemo liječenje, te pravi lijek mora balansirati između ublažavanja boli s jedne strane te drugih aspekata, kao što su psihički, emocionalni, sociološki te ponekad vjerski razlozi. Do danas su tehnika, instrumenti i lijekovi napredovali te se epiduralna analgezija počinje sve češće koristiti. U ovom radu ćemo razmotriti koja je ciljna terapija za bol u porodu te kako se epiduralna analgezija uspješno svrstava među najbolja i najuspješnija rješenja.

Abstract (english)

Even today, for most pregnant women, delivery causes severe pain, similar in degree to that caused by complex regional pain syndromes or the amputation of the finger. Although severe pain is not life-threatening in healthy parturient women, it can have neuropsychological consequences. Postnatal depression may be more common when analgesia is not used, and pain during labour has been correlated with the development of post-traumatic stress disorder.
Numerous strategies, both pharrmacologic (systemic opioids began to use 1840.- and to date have become the most commonly used drugs) and nonpharmacologic (TENS, massage, water bath, acupuncture, hypnosis) have been used as a treatment. However, childbirth is a multidimensional experience and when considering treatment, one must balance between pain relief and other aspects, such as physical, emotional, psychological, sociologic, and sometimes religious considerations. In this study we will consider the goals of therapy for labor pain and how neuraxial analgesia helps to accomplish these goals.
