Abstract

Akne je upalna dermatoza koja pogađa velik broj adolescenata. Nakon akne mogu zaostati ožiljci koji predstavljaju veliki estetski problem za bolesnike. Ožiljci nakon akne rezultat su poremećaja složenih mehanizama u procesu cijeljenja tkiva koji prolazi kroz upalnu, granulacijsku fazu i fazu remodeliranja. Nastanak ožiljaka u korelaciji je s trajanjem i težinom upale u lezijama od akne. Prema odgovoru tkiva na upalu razlikuju se hipetrofični ožiljci i keloidi te atrofični ožiljci. Atrofični ožiljci se dijele na icepick, rolling i boxcar ožiljke. U liječenju ožiljaka nakon akne koristi se velik broj metoda koje uključuju različite oblike resurfacinga, upotrebu kožnih implantata te različite kirurške metode. U terapiji atrofičnih ožiljaka koriste se: kemijski peeling, ablativni i neablativni laseri, frakcionirana fototermoliza, dermoabrazija, mikrodermoabrazija, kožni implantati te kirurške metode uključujući „punch“ eksciziju, „punch“ elevaciju i subciziju. Metode za liječenje hipertrofičnih ožiljaka i keloida jesu intralezijsko injiciranje kortikosteroida, silikonske gel-pločice, laseri, krioterapija te kirurško odstranjenje. Odabir adekvatne metode ovisi o vrsti ožiljka i njegovim karakteristikama, no treba voditi računa i o individualnim karakteristikama pacijenta, prednostima i ograničenjima svake metode te o očekivanjima pacijenta. Postojećim metodama može se postići znatno poboljšanje u izgledu ožiljaka, no nije moguće postići njihovo potpuno uklanjanje. Rano i adekvatno liječenje akne, kojim se ograničava upala u lezijama od akne, najbolja je metoda za prevenciju ili ublažavanje ožiljaka.

Abstract (english)

Acne is inflammatory dermatosis which has high prevalence among adolescents. Possible outcome of acne are acne scars which represent large esthetic problem for such patients. Acne scars result from disturbances in the complex mechanisms of wound healing which progresses through inflammation, granulation tissue formation and matrix remodeling. Scar formation is in relation with severity and duration of inflammation in acne lesions. According to the tissue response to inflammation, there are hypertrophic scars, keloids and atrophic scars. Atrophic scars have been classified into icepick, rolling and boxcar scars. There are many therapeutic methods available for the treatment of acne scarring, including resurfacing techniques, dermal fillers and different surgical methods. Treatment for atrophic scars include: chemical peeling, ablative and nonablative lasers, fractional photothermolysis, dermoabrasion, microdermoabrasion, dermal fillers and surgical methods including punch excision, punch elevation and subcision. Methods for hypertrophic scars and keloids include: intralesional steroid therapy, silicone gel, laser, cryotherapy and surgery. Choosing the adequate method depends on the scar type and its characteristics, but individual characteristics of a patient, advantages, limitations of each method and expectations of patients should also be considered. Current therapeutic methods may achieve significant improvement in scar appearance, but none of it can achieve complete resolution of a scar. Early and adequate acne treatment, which minimizes inflammation in acne lesion, is the best method for prevention and moderation of acne scars.
