Abstract | Prijelomi kostiju česta su i svakodnevna medicinska problematika. Nepravilno inicijalno
liječenje prijeloma može voditi do značajnog i dugotrajnog obolijevanja te potencijalno i
do smrti. Trauma je uzrok preko 140,000 smrti godišnje u Sjedinjenim Američkim
Državama, vodeći je uzrok smrti populacije od 1-34 godine te uzrokuje gubitak više
godina produktivnosti u populaciji do 65 godina negoli koronarna bolest, moždani udar i
maligne bolesti zajedno. Prema procjenama WHO-a ozljede zauzimaju udio od 12% svih
godina života prilagođenih na onesposobljenost i godina izgubljenih zbog prerane smrti
(DALY), u što je uključen i znatan broj prijeloma. Svrha ovog preglednog rada je
sumiranje znanja i principa patofiziologije prijeloma te operativnog liječenja prijeloma.
U prvom se dijelu opisuju mehanika prijeloma kosti, klasifikacije prijeloma koje su u
uporabi i na kraju fiziološko cijeljenje kostiju. Drugi dio je podijeljen na dva dijela. U
prvom se opisuju osnovni principi unutarnje fiksacije kosti. Unutarnja fiksacija
podrazumijeva fiksaciju vijcima, pločicama i intramedularnim čavlima te omogućava i
pospješuje cijeljenje. Drugi dio opisuje osnovne principe vanjske fiksacije. Treći dio se
sastoji od diskusije i zaključaka o tome koji operativni princip daje najbolji rezultat u
odnosu na tip frakture te koja je kost zahvaćena. |

Parallel abstract (English) | Orthopedic fractures are a common daily acute health issue. Improper initial
management of fractures can lead to significant long-term morbidity and, potentially,
mortality. Trauma causes more than 140,000 deaths per year in the United States, is the
leading cause of death for those aged 1-34 years, and causes more years of lost
productivity before age 65 years than coronary artery disease, cancer, and stroke
combined. The WHO estimated that injuries account for 12% of all disability-adjusted life
years (DALYs) lost, which includes a significant number of fractures. The main purpose
of the paper is to sum up the knowledge and principles of fracture pathophysiology and
fracture operative treatment. The first part describes bone fracture mechanics,
classifications of fractures that are in use and finally physiological fracture healing. The
second part is subdivided into two parts. Part one explains basic principles of internal
bone fixation. Internal fixation refers to fixation of screws and/or plates, intramedullary
bone nails to enable or facilitate healing. Second part explains basic principles of
external bone fixation. The third part consists of discussion and draws conclusions which
operative principle gives best results in relation to the type of fracture and the bone that
is fractured. |
