Abstract

Sportsko srce skup je kliničkih, elektrokardiografskih i ultrazvučnih promjena na srcu najčešće primijećenih kod profesionalnih sportaša čije bavljenje sportom uključuje dugotrajnu, primarno aerobnu, tjelesnu aktivnost. Riječ je o sindromu koji objedinjuje sinusnu bradikardiju u mirovanju, sistolički šum te povećanje srčanih šupljina. Kao benigni strukturni i funkcionalni proces prilagodbe rezultat je fiziološke srčane hipertrofije povezane s normalnom ili poboljšanom srčanom funkcijom što je i glavna razlika prema patološkim hipertrofijama srčanog mišića kao posljedici podležeće strukturne bolesti. Pri tome najveću dijagnostičku dilemu predstavlja hipertrofijska kardiomiopatija čiju važnost dodatno naglašava to što je i jedan od najčešćih uzroka iznenadne srčane smrti u mladih sportaša. S ciljem ranog otkrivanja takvih i sličnih stanja provode se preventivni pregledi sportaša kako bi se na vrijeme ograničila tjelesna aktivnost čiji intenzitet može biti okidačem fatalnih aritmija poput tahikardije i fibrilacije ventrikula koje posljedično dovode do iznenadne srčane smrti, često prvog i jedinog simptoma postojeće neprepoznate bolesti. Nasuprot tome, bitno je imati na umu postojanje fiziološke prilagodbe kako ne bi došlo do preranog prekida karijere zdravog sportaša. Usprkos pozitivnom sadašnjem stavu o sportskom srcu, postavlja se pitanje mogućih dugoročnih posljedica strukturne promjene srca u starijoj životnoj dobi što ostavlja otvoren prostor za dodatne prospektivne kohortne studije.

Abstract (english)

Athlete’s heart is a constellation of clinical, electrocardiographic, and echocardiographic findings on the heart mainly noticed in athletes engaged in long-term, primarily aerobic, sport activities. It is a syndrome that includes sinus bradycardia at rest, systolic murmur, and cardiac chamber enlargement. As a benign structural and functional adaptational process, it is a result of physiological cardiac hypertrophy associated with normal or augmented cardiac function unlike pathological hypertrophy, as a result of underlying structural disease. In that context hypertrophic cardiomyopathy represents the biggest diagnostic dilemma as one of the most common causes of sudden cardiac death in young athletes. With the aim of early detection of that and other similar diseases, pre-participational screening in athletes is conducted to limit the intensity of physical activity that can lead to fatal arrhythmias such as ventricular tachycardia or fibrillation, which can cause sudden cardiac death as the first and only symptom of an existing unrecognized disease. On the other hand, it is important to have in mind that physiological adaptation also exists, so that sport careers of healthy athletes wouldn’t be prematurely terminated. Despite current positive attitudes towards athlete’s heart, long-term consequences of structural heart changes in elderly age are questionable, which leaves a space for additional prospective cohort studies.
