Abstract

Već desetljećima stimulacija srca je jedna od djelotvornih terapijskih opcija u ljudi sa poremećajima srčane frekvencije. U tu svrhu se najviše ugrađuje dvokomorni elektrostimulator srca. Međutim u dosadašnjim studijama su se opisivali štetni učinci stimuliranja desne klijetke i njezine posljedice u određenim patološkim stanjima. Abnormalna provodljivost električnog impulsa i nepravilna mehanička kontrakcija srca su predstavljeni kao glavni štetni faktori pri stimulaciji desne klijetke koji dovode do remodeliranja miokarda. Remodeliranje miokarda je skup promjena koje uključuju promjenu funkcije i strukture samoga srca. Za praćenje promjena i evaluaciju učinka stimulacije srce koristimo se najčešće metodama ehokardiografije iz koje možemo izmjeriti sve potrebne parametre. U ovome radu su razmotreni štetni učinci stimulacije desne klijetke tj. asinkronija kao najvažnija posljedica stimulacije desne klijetke. Osim toga razmotrene su i uspoređene današnje terapijske metode kojima se takav učinak izbjegava (stimulacija pretklijetke pri normalnoj provodljivosti kroz AV čvor, trokomorna elektrostimulacija srca i eventualna stimulacija LK ili Hisovog snopa).

Abstract (english)

For decades, heart stimulation is one of the most effective therapeutic options in people with heart rate disorders. For this purpose, dual chamber pacemaker is most commonly used. However, previous studies have described the adverse effects of right ventricular pacing and its consequences in certain pathological conditions. Abnormal electrical conduction of impulse and improper mechanical contraction of the heart are presented as the main adverse factor in right ventricular pacing which causes cardiac remodeling. Cardiac remodeling is set of changes that involve changing the function and structure of the heart itself. To monitor and evaluate the effect of heart stimulation, we use echocardiography as the most common method from which we can measure all the necessary parameters. This paper discusses the adverse effects of right ventricular pacing, i.e. asynchrony as the most important consequences of right ventricular pacing. In addition, we reviewed and compared today’s therapeutic methods to avoid right ventricular pacing effect (right atrium pacing at normal AV node conductivity, CRT and eventual pacing of LV or bundle of His).
