Abstract

Konvencionalna mehanička ventilacija novorođenčadi postupak je koji omogućuje umjetnu potporu izmjeni plinova kada se ta izmjena spontanim disanjem ne može osigurati, a izvodi se pomoću posebnih uređaja – ventilatora. Takva ventilacijska potpora invazivni je postupak za koji treba imati jasne indikacije i ciljeve takva liječenja. Poremećaji disanja predstavljaju najčešća i najteža patološka stanja u novorođenačkoj dobi. Za sigurnu i uspješnu umjetnu ventilaciju novorođenčadi potrebno je poznavanje osnovnih postulata plućne fiziologije i patofiziologije u novorođenačkoj dobi, kao i osnovniih modaliteta različitih oblika konvencionalne mehaničke ventilacije. Iskustan kliničar treba znati procijeniti utjecaj ventilacije na sistemsku cirkulaciju i rad srca, kao i razumjeti osnovne postupke kojima se može utjecati na izmjenu plinova, a s ciljem održavanja vrijednosti 𝑃𝑂2 i 𝑃𝐶𝑂2 u željenim granicama. Isto tako, način ventilacije treba dizajnirati prema karakteristikama pluća bolesnog novorođenčeta i patofoziološkom procesu koji uvjetuje respiratornu insuficijenciju, a koji se mijenja tijekom liječenja. Promjene je najbolje pratiti kontinuiranim monitoriranjem plućnih funkcija. Budući da je duljina primjene mehaničke ventilacije u korelaciji s nastankom komplikacija, vremenom provedenim u bolnici i cjelokupnim ishodom liječenja, monitoriranje pomaže i u procjeni trenutka kada se odvajanje novorođenčeta od respiratora čini mogućim. Odvajanje se ne bi trebalo nepotrebno odlagati jer mehanička ventilacija i nadalje ostaje vrlo invazivni postupak u liječenju novorođenčadi.

Abstract (english)

Conventional mechanical ventilation in neonates is a procedure that provides (allows) artificial support to gas exchange when this exchange cannot be ensured by spontaneous breathing. It is perfomed using a special device – the ventilator. This kind of ventilation support is an invasive procedure for which there should be clear indications and goals. Breathing disorders are the most common and the most severe pathological disorders in newborn infants. For safe and successful mechanical ventilation of newborns it is necessary to know the basic postulates of respiratory physiology and pathophysiology in the newborn age, as well as the basic model of various forms of conventional mechanical ventilation.
An experienced clinician should be able to evaluate the effect of ventilation on systematic circulation and heartbeat, as well as understand the basic procedures that can influence the gas exchange in order to maintain the values of 𝑃𝑂2 and 𝑃𝐶𝑂2 within desired limits. Likewise, the ventilation mode should be designed according to the characteristics of the lungs of the sick newborn and the pathophysiological process that causes respiratory failure, which changes during treatment. Changes are best followed by continuous monitoring of the pulmonary function. Since the duration of mechanical ventilation is in correlation with complications, time spent in the hospital, and the overall outcome, monitoring also helps estimate the moment of weaning from mechanical ventilation. Weaning should not be unnecessarily delayed, as mechanical ventilation remains a highly invasive procedure in the treatment of newborns.
