Abstract

Bolest bubrega česta je komplikacija nakon transplantacije jetre (TJ) i u velikoj mjeri doprinosi povećanom morbiditetu i mortalitetu transplantiranih bolesnika. Bubrežno oštećenje nakon TJ može se prezentirati u akutnom ili kroničnom obliku, podloga bolesti je multifaktorijalna te uključuje brojne preoperativne, perioperativne i postoperativne faktore. Poznavanje uzročnih faktora kao i rana identifikacija bubrežnog oštećenja doprinosi pravovremenoj primjeni terapije kao i sprečavanju progresije bolesti. Incidencija akutne bubrežne insuficijencije (ABI) nakon TJ kreće se u rasponu od 40-70%, najčešće se javlja se u ranom postoperativnom razdoblju te je uzrokovana akutnom tubularnom nekrozom uslijed hipotenzije i hipoperfuzije. Prolongirana ishemija, toksična oštećenja, infekcije i nefrotoksični lijekovi faktori su koji mogu dovesti do progresije ABI-e u ireverzibilno, kronično oštećenje. Kronična bubrežna bolest (KBB) predstavlja značajan problem nakon TJ s incidencijom koja se kreće od 20-80%. Među brojnim faktorima rizika za razvoj KBB vodeću ulogu imaju kalcineurinski inhbitori (CNI) koji su sastavna komponenta većine imunosupresijskih protokola. S obzirom na sve veću incidenciju bubrežnih bolesti nakon TJ, pravovremeno prepoznavnje bolesnika s povišenim rizikom za razvoj bubrežnog oštećenja, identificiranje etioloških faktora, kao i pravodobna primjena renoprotektivne strategije, uključujući minimaliziranje CNI nefrotoksičnosti, važni su postupci u očuvanju bubrežne funkcije nakon TJ.
Ovaj rad donosi pregled mogućih bubrežnih bolesti nakon TJ s osvrtom na incidenciju, etiologiju i rizične faktore, posljedične komplikacije te strategije za očuvanje i minimaliziranje bubrežnih oštećenja nakon TJ.

Abstract (english)

Kidney disease after liver transplantation is a common complication associated with increased morbidity and mortality. It may have an acute or chronic presentation. Aetiology is multi-factorial, including many preoperative, perioperative and postoperative factors. Identifying risk factors and early identification of renal dysfunction may allow early implementation of treatments that could delay the progression of renal damage after liver transplantation.
Acute kidney injury occurs in early postoperative period at an incidence of 40- 70% and its most common cause is acute tubular necrosis due to hypotension and hypoperfusion. Prolonged ischemia, toxic insults, infections and nephrotoxic drugs may contribute to progression of acute kidney injury into ireversible, chronic kidney disease. Incidence of chronic kidney disease after liver transplantation ranges from 20 to 80%, and it is infulenced by many different factors. However calcineurin inhibitors nephrotoxicity is one of the leading causes.
Identifying patients at risk of acute kidney injury and chronic kidney disease and early implementation of measures to preserve the progression of renal dysfunction, including CNI minimization, should be an integral part in renoprotective strategies.
This paper provides an overview of kidney diseases occuring liver transplantation, including incidence, aetiology and risk factors, following complications and strategies to preserve and reduce renal dysfunction after liver transplantation.
