Abstract

Bolesti pluća i dišnih puteva jedne su od najčešćih bolesti današnjice. Njihov raspon je od blagih, samoograničavajućih prehlada do teških, po život opasnih promjena na plućima koje zahtijevaju medikamentoznu ili operativnu terapiju. Promjene dišnih puteva i pluća dovode do promjena u funkciji koje predstavljaju izazov tijekom perioperativnog i postoperativnog razdoblja. Stoga je bitno da se provede temeljita preoperativna obrada takvih bolesnika, stavljajući naglasak na anamnezu i fizikalni pregled. Time se smanjuje postoperativni morbiditet i povećava se pozitivni ishod postupka. Svaki znak bolesti (dispneja, kašalj, iskašljaj), mogao bi otkriti asimptomatskog ili pogoršanje u postojećem stanju već poznatog bolesnika te time i indikacije bilo za poboljšanje stanja dišnih puteva bolesnika bilo za odgodu operacije. Ako se ukaže potreba, može se posegnuti za dodatnim pretragama koje bi utvrdile postojeće stanje pacijenta te time pokazale da li je pacijent u stanju podnijeti operativni postupak ili se on mora otkazati. Brojna stanja mogu utjecati na ishod operativnog postupka te je zadatak preoperativne pripreme da ukaže na njih kako bi se mogao stvoriti adekvatni anesteziološki plan za svakog pacijenta. Takav način individualizacije preoperativne pripreme, ne samo da povećava pozitivni ishod za samog pacijenta, smanjuje morbiditet i mortalitet, vrijeme operacije, cijenu, otkazivanje ili odlaganje zahvata.

Abstract (english)

Lung and airway diseases are one of the most common diseases today. They range from mild, self-limited common cold to the severe, life threatening lung changes that demand medicament or operative management. Changes in the airway and lungs lead to changes in the function which can make difficulties during the perioperative and postoperative time. Therefore it is important to conduct thorough preoperative assessment of these patients, especially anamnesis and physical examination. Every sign of the disease (dispnea, cough, sputum), could discover an asymptomatic or worsening of condition of already known patient and with that indications for either an improvement of the airway condition or an operation delay. If necessity occurs, additional examinations could be performed in order to confirm current condition of the patient and therefore reveal if the patient is eligible for operative procedure or should the procedure be delayed. Numerous conditions can affect the results of operative procedure and it is the task of preoperative assessment to detect these conditions in order to create proper anesthesiological plan for every patient. Not only does such individual approach in preoperative assessment increases positive outcome for the patient but it also decreases mortality, time under the operative procedure, cost and delays.
