Abstract

Depresivni poremećaj spada među najčešće poremećaje u općoj populaciji. Procjenjuje se da je čak oko 4% populacije u svakom trenutku zahvaćeno depresijom. Zdravstveni stručnjaci s razlogom tvrde da je epidemija depresije na pomolu i da će depresija unutar nekoliko desetljeća postati jedan od najčešće dijagnosticiranih poremećaja. Kemijski spojevi monoaminske strukture (monoamini, kao što su serotonin, noradrenalin i dopamin) imaju važnu ulogu u patofiziologiji depresije. Antidepresivi su lijekovi koji se koriste u liječenju simptoma depresije i prevenciji budućih relapsa. Postižu svoj efekt modulirajući neurotransmisiju spomenutih monoamina. Nekoliko je klasifikacija antidepresiva u upotrebi. Najkorisnija je podjela po djelovanju na monoaminski sustav. Ta klasifikacija dijeli antidepresive u sljedeće skupine: selektivni inhibitori ponovne pohrane serotonina (SSRI/SIPPS), selektivni inhibitori ponovne pohrane serotonina i noradrenalina (SNRI), triciklički antidepresivi (TCA), inhibitomonoaminooksidaze (MAOI), inhibitori ponovne pohrane noradrenalina i dopamina (NDRI), noradrenergički i specifični serotoninergički antidepresivi (NASSA) i mnoge druge. Budući da se svi antidepresivi ne ponašaju jednako, čak ni unutar iste skupine, od izuzetne je važnosti poznavati sličnosti i razlike u njihovom djelovanju kako bi se odabrao pravi antidepresiv za bolesnika. Antidepresivi su svoje mjesto pronašli i u liječenju drugih poremećaja kao što su anksiozni poremećaji, PTSP, socijalna anksioznost, bulimija, panični poremećaji, OKP, kroničan bol i mnogi drugi.

Abstract (english)

Depressive disorders are among the most common disorders in general population. It is estimated that as much as 4% of the population is at some point affected by it. Health care professionals rightfully believe that a depression epidemic is on the rise and that it will become one of the most diagnosed diseases in the next few decades. Molecules with monoamine structure (i.e. serotonin, norepinephrine and dopamine) play a vital role in the pathophysiology of depression. Antidepressants are drugs used to treat the symptoms of depression and prevent future relapses. They exhibit those effects by modulating the aforementioned monoamines. There are several classifications in use, but the most useful one is according to their effects of the monoamine system. This classification divides antidepressants into: selective serotonin reuptake inhibitors (SSRI), selective norepinephrine and serotonin reuptake inhibitors (SNRI), tricyclic antidepressants (TCA), monoamine oxidase inhibitors (MAOI), norepinephrine and dopamine reuptake inhibitors (NDRI), noradrenergic and specific serotonergic antidepressants (NASSA) and many others. Because antidepressants exert different effects, even within the same group, it is of utmost importance to know the similarities and differences in their mechanism of action in order to find the right antidepressant for the patient. Antidepressants are also finding their place in treating other disorders such as anxiety disorders, PTSD, social anxiety, bulimia, panic disorders, OCD, chronic pain disorders and many more.
