Abstract | Akromegalija je teška sistemna bolest koja nastaje kao posljedica dugotrajnog djelovanja hormona rasta. Najčešći su uzroci pojave akromegalije funkcionalni tumori hipofize: mikroadenomi i makroadenomi. Zbog brojnih komplikacija vezanih uz akromegaliju, životni vijek bolesnika često je skraćen kao i njegova kvaliteta. Opstrukcijska apneja u spavanju (OSA) je najčešći respiratorni poremećaj koji se javlja kod oboljelih od akromegalije. Cilj ovog istraživanja bio je utvrditi učestalost pojave OSA-e i ustanoviti korelaciju s kliničkim obilježjima bolesti.
Ispitano je 26 bolesnika s akromegalijom (8 muškaraca, 18 žena, prosječna dob 50,1 ±13,8 godina). Za računanje apneja-hipopneja indeksa (AHI) korištena je cjelonoćna polisomnografija.
Od ukupno 26 bolesnika s akromegalijom njih 13 imalo je OSA-u od čega 8 teški oblik (AHI>30). Medijan indeksa AHI iznosio je 5,4. Samo 4 bolesnika imala su aktivnu bolest dok je njih 22 bilo u remisiji. Prosječno vrijeme trajanja simptoma do postavljanja dijagnoze iznosilo je 4,2 godine, a trajanje remisije 2 godine.
Vrijednosti indeksa AHI nisu bile povezane s aktivnošću bolesti, trajanjem bolesti, trajanjem remisije, vrijednostima BMI-a, dobi i spola bolesnika, prisutnošću hipertenzije, dijabetesa, dislipidemije, KOPB-a i pušenjem.
Rezultati istraživanja ograničeni su retrospektivnim dizajnom i malim brojem ispitanika. Istraživanje je pokazalo da je opstrukcijska apneja u spavanju čest nalaz kod oboljelih od akromegalije, čak i nakon postizanja remisije bolesti. |

Parallel abstract (English) | Acromegaly is a severe systematic disease which occurs as a result of long-term activity of growth hormone. Most common reasons of acromegaly are pituitary functional tumors: microadenoma and macroadenoma. Due to many complications related to acromegaly, both life expetancy and quality of life in patients with this disease are significantly reduced. Obstructive sleep apnea is the most common respiratory disorder in people with acromegaly. The aim of this study was to determine incidence of obstructive sleep apnea in people with acromegaly and establish correlation with the clinical features of the disease.
Twenty six patients with acromegaly diagnosis were examined (8 males,18 females,averege age 50,1 ±13,8 years). Overnight polysomnography was used for calculation of apnea -hypopnea index (AHI).
Thirteen out of twenty six patients had obstructive sleep apnea and 8 among them had severe type of disease (AHI>30). Median AHI value was 6. Only 4 patients had active disease while the other 22 patients were in remission. The average time span from onset of symptoms to diagnosis was 4,2 years and duration of remmision was 2 years.
The values of the AHI were not associated with disease activity, disease duration, remission duration, BMI values, age and sex of the patient, presence of hypertension, diabetes, dyslipidemia, COPD and smoking.
The study results are limited by the retrospective design and the small number of respondents.
This research showed that the obstructive sleep apnea is a common finding in patients with acromegaly even after the remission is achieved. |
