Abstract

Multimorbiditet predstavlja rastući problem u populaciji za koju skrbi liječnik obiteljske medicine (LOM). Zadatci liječnika i drugih zdravstvenih profesionalaca koji pružaju zdravstvenu zaštitu na primarnoj razini, osim liječenja i sprečavanja pogoršanja kroničnih bolesti, još su i rješavanje poteškoća u dnevnom funkcioniranju s kojima se suočavaju bolesnici s multimorbiditetom. Zbog progresivnog starenja stanovništva sve se više povećava broj bolesnika koji istovremeno pate od više kroničnih bolesti. Tome pridonosi ne samo starost populacije, nego i utjecaj dodatnih faktora kao što su: način života, socioekonomski status, genetika i biopsihosocijalne odrednice. Unatoč visokoj i rastućoj prevalenciji multimorbiditeta još uvijek ne postoje jasno definirane smjernice i oblici liječenja za takve bolesnike. Ono što je poznato je da multimorbiditet zahtijeva skrb koja nadilazi dosadašnje okvire skrbi za pojedine kronične bolesti i da su neizbježne promjene u sustavu koji liječi te bolesnike. Te promjene se, prije svega, odnose na organizaciju zdravstvene službe na primarnoj razini zaštite, posebice liječnika obiteljske medicine, ali i na promociju zdravijeg načina života među pacijentima i općenito promjeni njihovog stava vezanog za bolest u smislu samostalne skrbi i jačanja potpore od strane zajednice u kojoj žive. Postoji nekoliko modela skrbi za bolesnike s multimorbiditetom koji su nastali krajem prošlog stoljeća i koji se dan danas primjenjuju u praksi. Glavne odrednice tih modela su integrirana zdravstvena zaštita i izvaninstitucionalna skrb koja uključuje više članova tima, bilo medicinskih radnika, bilo neformalnih sudionika u skrbi za tog bolesnika. Zbog toga se nastoji izgraditi potporno socijalno okruženje za pacijenta, što pozitivno utječe na njegovo mentalno i fizičko stanje. Multimorbiditet bolesnicima predstavlja veliki životni teret jer su često opterećeni većim brojem lijekova kao i potrebom za brojnim posjetama liječniku obiteljske medicine, dijagnostičkim i terapijskim postupcima te specijalistima drugih specijalnosti. Uz napredovanje bolesti i starenje, nadovezuje se propadanje samostalnih funkcionalnih vještina, odnosno pojava i povećanje nemoći, te posljedično povećanje ovisnosti o pomoći okoline i zajednice.

Abstract (english)

Multimorbidity is a growing problem in family medicine clinics. Primary level health care helps with the problems that patients with multimorbidity must deal with. Because of the progressive aging of the population, the number of patients who can be classified in this group is increasing. This is not only due to the age of the population, but also to the influence of additional factors such as lifestyle, socioeconomic status, genetics and biopsychosocial determinants. Despite the high and growing prevalence of multimorbidity, there are still no clearly defined guidelines and forms of treatment for such patients. What is known is that multimorbidity requires care that emerges from the current care framework for some chronic diseases and that there are inevitable changes in the system that treats these patients. These changes relate primarily to the organization of the family medicine physician's healthcare, but also to the promotion of a healthier way of life among the patients and, in general, to change their attitude towards the illness in terms of self-care and the strengthening of the support of the community in which they live. There are several models of care for patients with multimorbidity that were created at the end of the last century and are still being applied in practice today. The main determinants of these models are integrated clinical and out-of-clinic care that includes multiple team members of either medical staff or informal participants in the care of that patient. Therefore, it is important to build a supportive social environment for the patient, which positively affects his mental and physical condition. Multimorbidity is a major burden to patients because they are often burdened with a large amount of medication and frequent doctor visits. In addition to this, the increase of frailty enhances the community dependency.
