Abstract

Subjektivni kognitivni poremećaj je stanje koje nosi povećani rizik konverzije u primarne neurodegenerativne demencije, prije svega Alzheimerovu bolest. Stoga osobe s tim poremećajem zahtijevaju kliničku obradu i kontinuirano praćenje. Radi se o subjektivnoj procjeni opadanja kognitivnog funkcioniranja koje se objektivno ne može potvrditi. Subjektivni kognitivni poremećaj može se pojaviti u različitim patološkim stanjima, ali često se veže uz blage oblike depresije. Otkrivanjem etiologije moguće je procijeniti tijek/ishod kognitivnog poremećaja. Upotrebom različitih slikovnih i neurobiokemijskih tehnika moguće je kod nekih osoba naći promjene tipične za Alzheimerovu bolest. Za Alzheimerovu bolest trenutno ne postoji uzročna terapija ili terapija koja modificira tijek bolesti koju bi bilo najbolje započeti davati u što ranijoj, pretkliničkoj fazi bolesti, no jednom kada takva terapija bude otkrivena prepoznavanje osoba sa subjektivnim kognitivnim poremećajem koji u podlozi imaju Alzheimerovu bolest će postati izuzetno važno.

Abstract (english)

Subjective cognitive impairment is a condition with an increased risk of conversion to primary neurodegenerative dementias, especially to Alzheimer's disease. Therefore, persons with this disorder require clinical evaluation and continuous monitoring. They report subjective decline in cognitive functioning which cannot be objectively confirmed. Different pathological conditions may present with subjective cognitive problems. Often it is associated with mild forms of depression. By revealing underlying etiology, it is possible to estimate potential of progression and outcome of cognitive impairment. Several imaging and neurobiochemical biomarkers enable detection of underlying Alzheimer's disease pathology in persons with subjective cognitive impairment. Causal or disease-modifying therapy for Alzheimer's disease is still missing. Since it would be the best to start this treatment in the earliest, preclinical stage of disease, identifying persons with subjective cognitive impairment with underlying Alzheimer's disease will become extremely important.
