| Abstract | Akromegalija je rijetka i kronična bolest koja nastaje kao posljedica prekomjernog lučenja hormona rasta. Zahvaća populaciju srednje životne dobi čije su epifizne ploče kostiju zatvorene. U podlozi se najčešće nalazi tumor hipofize makroadenom (>10mm) ili mikroadenom (<10mm) koji pretjerano izlučuje hormon rasta. Klinička slika može dugo ostati neprepoznata te se ispoljiti tek nakon 10 i više godina. Pacijenti se najčešće prezentiraju povećanjem šaka i stopala, zadebljanjem mekog tkiva ruku, kraniofacijalnim promjenama, te brojnim komorbiditetima od kojih su najznačajniji hipertenzija, opstruktivna sleep apneja, dijabetes mellitus tip II, artropatija te benigne i maligne tvorbe na crijevima i štitnoj žlijezdi. Dijagnoza se postavlja na temelju anamneze i kliničke slike, oralnog glukoza tolerans testa, laboratorijskih nalaza vrijednosti hormona rasta i inzulinu sličnog faktora rasta-I, te slikovnim metodama. Zlatni standard u liječenju je transsfenoidalna operacija, a ostali modaliteti su medikamentna i radioterapija.
Prethodna istraživanja su pokazala da hipertenzija, dijabetes mellitus tip II, hiperlipidemija, opstruktivna sleep apnea, artropatija, benigne i maligne promjene probavnog sustava i štitnjače spadaju u najzastupljenije komorbiditete. Cilj ovog rada bio je retrospektivnom analizom medicinske dokumentacije oboljelih od akromegalije ustanoviti učestalost i pojavnost komorbiditeta.
Primjenom brojnih dijagnostičkih metoda ispostavilo se da najučestaliji komorbiditeti su benigne i maligne promjene štitnjače (72,7%) opstruktivna sleep apnea (61,9%), hipertenzija (61%), smetnje vida (41,2%) polipi debelog crijeva (40,8%) dijabetes mellitus tip II (33,1%), hiperlipidemija (21,3%), što se podudara sa otprije opisanim rezultatima. |
| Abstract (english) | Acromegaly is a rare and chronic disease that arises as a result of excessive growth hormone hypersecretion. It affects middle aged population in which growth plate has already been closed. The most common cause is pituitary gland tumor either macroadenoma (>10mm) or microadenoma (<10mm) which excessively excrete growth hormone. Signs and symptoms are usually unrecognized even for ten years or more. Patients most commonly present with hand and feet enlargement, soft tissue thickening of hands, craniofacial changes, and various comorbidities such as hypertension, obstructive sleep apnoea, diabetes melltus type II, arthropathy and benign and malignant formations in gut and thyroid gland. Diagnosis is based on thorough medical history and physical examination, oral glucose tolerance test, laboratory findings of excessive growth hormon, insulin-like growth factor I and radiological imaging. Transsphenoidal surgery is golden standard in treating patients suffering from acromegaly, other methods of treatment are with medications and radiotherapy.
Previous research showed that hypertension, diabetes mellitus type II, hyperlipidemia, obstructive sleep apnoea, arthropathy, benign and malignant formations in gut and thyroid gland, are the most common comorbidities. The aim of this paper was to retrospectively analyse medical history and determine frequency and occurrence of comorbidities in patients affected with acromegaly.
The results showed that most common comorbidities are benign and malignant changes of thyroid gland (72,7%), obstructive sleep apnoea (61,9%), hypertension (61%), visual impairment (41,2%), colon polyps (40,8%), diabetes mellitus type II (33,1%), hyperlipidemia (21,3%) which coincides with previous research. |
