Abstract

Rak tijela maternice najčešći je rak ženskog genitalnog sustava u razvijenim zemljama. U Hrvatskoj se nalazi na četvrtom mjestu po učestalosti. Najčešće obolijevaju žene u postmenopauzi (prosječna dob 62 godine). Najčešći histološki oblik je adenokarcinom. Kirurško određivanje proširenosti bolesti, tzv. kirurški staging, uvršten je u standardni postupak u bolesnica s rakom endometrija. Uključuje, uz odstranjenje maternice i adneksa, uzimanje slobodne tekućine za citološku analizu (ispirak) te histološku analizu regionalnih limfnih čvorova. Kirurško određivanje bolesti omogućava određivanje prisutnosti ili odsutnosti materničnih i vanmaterničnih rizičnih faktora. Maternični rizični čimbenici uključuju histološki tip tumora, gradus tumora, dubinu invazije tumora, limfokapilarnu invaziju i invaziju vrata maternice. Vanmaternični rizični čimbenici uključuju zahvaćenost zdjeličnih i paraaortalnih limfnih čvorova. Cilj ovog diplomskog rada je istražiti utjecaj prognostičkih čimbenika kod oboljelih od raka endometrija na pojavnost recidiva bolesti. Ispitanice su podijeljene u dvije skupine; pacijentice s recidivom bolesti i bez recidiva bolesti. Recidiv bolesti razvilo je približno 10% ispitanica. Statističkom analizom podataka pokazalo se da na pojavnost recidiva kod oboljelih od raka endometrija utječu sljedeće varijable: dubina invazije tumora (mm), limfokapilarna invazija, gradus tumora, zahvaćenost limfnih čvorova, invazija istmusa i invazija vrata maternice.

Abstract (english)

Carcinoma of the endometrium is the most common female genital system malignancy in developed countries. In Croatia, it's the fourth most common cancer in females. This disease occurs mostly in postmenopausal women (mean age, 62 years). The most common histological type is adenocarcinoma. Surgical estimation of the stage of the disease, surgical staging, is a standard procedure with endometrium cancer patients. It includes uterus and adnexa removal, obtaining fluids for cytologic evaluation and histological examination of lymph node tissues. Surgical staging enables determination of presence or absence of uterine or extrauterine risk factors. Uterine risk factors are: hystological cancer type, carcinoma invasium gradus, depth of tumor invasion, lymphovascular space invasion and cervical cancer invasion. Extrauterine risk factors are pelvic and paraaortic lymph nodes. The main goal of this thesis is to explore the impact of the prognostic factors in patients with endometrial cancer on cancer recurrence. Female examinees were classified into two groups: patients with cancer recurrence and patients without cancer recurrence. Approximately 10 % of the examinees developed recurrence. Statistical data analysis has shown that the following variables have an impact on cancer recurrence: depth of tumor invasion (mm) and lymphovascular, lymphatic system, isthmus, and cervical invasion.
