Abstract

Diljem svijeta debljina je veliki javnozdravstveni problem koji je poprimio pandemijske razmjere. Definiramo ju kao prekomjerno nakupljanje masnog tkiva u organizmu, odnosno povećanje indeksa tjelesne mase iznad 30,00 kg/m². U Hrvatskoj u tu kategoriju možemo svrstati jednu petinu stanovnika, a u svijetu se bilježi preko 500 milijuna osoba s debljinom. Debljina povećava rizik za oboljevanje od mnogih bolesti, kao što su šećerna bolest tip2, hipertenzija ili neka sijela tumora. Osim toga ekonomski teret debljine procjenjuje se na oko 10% ukupnih troškova zdravstvene zaštite. Stres je neugodno stanje napetosti praćeno tjelesnim i psihičkim promjenama koje nastaju kao rezultat reakcije na događaje, situacije ili stanja koja osoba procjenjuje opasnim ili ugrožavajućim za svoj psihički i/ili tjelesni integritet. Rizičan je faktor za razvoj kardiovaskularnih bolesti, te pogoršava tijek već postojećih kroničnih bolesti, kao što su šećerna bolest tip2, astma i kardiovaskularne bolesti. Sve je više istraživanja koja pokazuju da postoji jasna povezanost između stresa i debljine. Mehanizam kojim stres utječe na povećanje tjelesne mase je kompleksan i još nije do kraja razjašnjen. Primarno on djeluje kroz promjenu osi hipotalamus-hipofiza-nadbubrežna žlijezda koji je dijelom reguliran i spolnim hormonima, što objašnjava razliku u odgovoru na stres među spolovima. Nađena je i povezanost stresa s emocionalnom neuralnom mrežom te tako preko konzumiranja hrane za utjehu (»comfort food«) emocije imaju utjecaj na debljanje. Povećan rizik za razvoj debljine nađen je među pripadnicima nižeg socioekonomskog statusa, i u odrasloj i u dječijoj dobi, sa sve više dokaza da je stres uzrok tome.

Abstract (english)

Obesity is a major public health problem that has taken on pandemic proportions around the world. We define it as excessive accumulation of adipose tissue in the organism, respectively increase of body weight index over 30,00 kg/m². In the Republic of Croatia within this category we can place one fifth of the population whereas the world registers over 500 million people with obesity. Obesity increases the risk of many diseases such as diabetes, hypertension or malignant disease. Apart from this the economic burden of obesity is estimated to be at around 10% of total health care costs. Stress is unpleasant state of tension followed by physical and mental changes that arise as a response to events, situations or states that a person may estimate as dangerous or threatening for its mental and/or physical integrity. It is a risk factor for the development of cardio-vascular diseases and it deteriorates the flow of existing chronicle diseases such as diabetes, asthma and cardio-vascular diseases. There are more and more studies that show the clear connection between stress and obesity. Mechanism by which stress influences increase of body weight is complex and still not completely explained. Primarily it operates through modification of hypothalamus-pituitary-adrenal axis by which is partially regulated by sex hormones, that explains the difference in response to stress between sexes. Connection of stress to emotional neural network has been identified and in this manner throughout comfort food consummation, emotions have also influence on obesity. Increased risk for the development of obesity is registered within representatives of lower socio-economic status, in adulthood and childhood with more and more evidence that it is caused by stress.
