Abstract

Sekretorni otitis smatra se jednom od najučestalijih pedijatrijskih bolesti. Manifestira se prisutnošću sekreta unutar bubnjišta bez znakova akutne upale uha. Najčešće se javlja u djece predškolske dobi. Smatra se da oko 60% djece ima barem jednu epizodu sekretornog otitisa do 2. godine života. Najčešća manifestacije jest konduktivni gubitak sluha koji može iznositi i do 50 dB. Od ostalih simptoma navode se osjećaj nelagode i punoće u uhu, pucketanje, dok se bol rijetko javlja. Kao glavni čimbenici koji dovode do nastanka ove bolesti smatraju se disfunkcija Eustahijeve cijevi i upale uha, a nastanku pridonose i niz drugih čimbenika kao što su alergije, kraniofacijalne malformacije te gastroezofagealni refluks. Dijagnoza se postavlja na temelju anamneze, otoskopije, timpanometrije i audiometrije. Otoskopski nalaz uključuje promjenu boje bubnjića ili nalaz tekućine te aerolikvidnih nivoa, dok timpanometrijski nalaz pokazuje zaravnjenu krivulju tipa B. Audiometrijski nalaz potvrđuje prisutnost konduktivnog gubitka sluha i ukazuje na to koliki je taj gubitak. Nakon postavljanja dijagnoze preporuka je da se pacijent prati tokom tri mjeseca zbog velike učestalosti spontanog prolaska. Nakon tri mjeseca potrebno je ponovno provesti evaluaciju stanja nakon čega se odlučuje o potrebi
daljnjeg liječenja. Kao glavna metoda liječenja preporučuje se kirurško liječenje, odnosno umetanje ventilacijske cjevčice s ili bez adenoidektomije.

Abstract (english)

Otitis media with effusion is considered as one of the most common diseases in pediatric population. It is manifested by the presence of secretion in the tympanic cavity without signs of acute infection. This condition usually occures in preschool-aged children and it is estimated that about 60% of children experience at least one episode of otitis media with effusion by the age of two. The most common manifestation is conductive hearing loss up to 50 dB. Other reported symptoms include a feeling of discomfort and fullness, crackling noises, while pain rarely occurs. Eustachian tube dysfunction and ear infection are considered as the major etiologic factors in the development of otitis media with effusion. Other contributing factors include allergy, craniofacial malformations and gastric reflux. Diagnosis is based on patients history, otoscopic, tympanometric and audiometric findings. Otoscopic findings include tympanic membrane discoloration, fluid or air-fluid levels, while tympanometric finding include flat B tipe curve. Audiometry is performed to determine type and size of hearing loss. Once the diagnosis is made, a 3-month period of observation is recommended due to self-limited nature of most cases. After the 3-month period re-evaluation is recommended after which a decision on further treatment is made. Surgical treatment is widely accepted therapeutic intervention for persistent otitis media with effusion, and include tube insertion with or without adenoidectomy.
