Abstract

Krutost velikih arterija je biljeg, ali i nezavisni čimbenik ubrzanog vaskularnog starenja i povećanoga kardiovaskularnog rizika. Prisutnost kroničnih upalnih bolesti dovodi do ubrzane ateroskleroze, ubrzanog vaskularnog starenja i povišenog kardiovaskularnog rizika što je uzrokovano kroničnom, sistemskom upalom, endotelnom disfunkcijom, duljinom trajanja bolesti te utjecajem različitih lijekova. Ateroskleroza je često prisutna u obliku subkliničke ateroskleroze, povećane debljine intima-medije ili krutosti arterija. Tradicionalni faktori rizika ne odražavaju u potpunosti povišeni kardiovaskularni rizik u ovoj populaciji. Stoga postoji sve veći interes za identifikacijom novih faktora rizika kako bi se objasnio rani razvoj endotelne disfunkcije i raniji nastanak ateroskleroze. Istraživanja snažno ukazuju na to da je ateroskleroza u velikoj mjeri pokretana upalom i aktivnim imunološkim odgovorom, a upućuju i na samu bolest kao neovisni faktor rizika. Aktivnost bolesti i samo trajanje bolesti dodatno povisuje rizik od nastanka kardiovaskularnih incidenata. Istraživanja su pokazala da su brzina pulsnoga vala i krutost arterija u većini kroničnih upalnih bolesti neovisno povezani s pojavnosti ubrzane ateroskleroze i posljedično veće krutosti arterija. Unatoč sve više radova,
još uvijek nije dokazana neovisna povezanost same bolesti, duljine trajanja i aktivnosti sa ubrzanim vaskularnim starenjem. Mjerenje krutosti arterija, određivanje proširenosti ateroskleroze i procjena kardiovaskularnoga rizika postaju klinički važni gdje je pažljivo praćenje i korekcija faktora rizika od presudne važnosti posebno u bolesnika s duljim trajanjem bolesti.

Abstract (english)

Arterial stiffness is a marker but also an independent factor for accelerated vascular aging and increased cardiovascular risk. The presence of chronic inflammatory diseases leads to development of accelerated atherosclerosis, accelerated vascular aging and increased cardiovascular risk, which is caused by chronic, systemic inflammation, endothelial dysfunction, duration of disease, and the impact of various drugs. Atherosclerosis is often present in the form of subclinical atherosclerosis, increased intima-media thickness, or arterial stiffness. Traditional risk factors do not fully reflect the elevated cardiovascular risk in this population. Thus, there is increasing interest in identifying novel risk factors to explain the early development of endothelial dysfunction and the early onset of atherosclerosis. Published data strongly suggests that atherosclerosis is largely driven by inflammation and an active immune response, and they point to the disease itself as an independent risk factor. Disease activity and the duration of the disease itself further increase the risk of cardiovascular events. Studies have shown that pulse wave velocity and arterial stiffness in most chronic inflammatory diseases are independently associated with the occurrence of accelerated atherosclerosis and, consequently, increased arterial stiffness. Despite novel findings, the independent association of the disease itself, its duration and activity with accelerated vascular aging has not yet been demonstrated. Measurement of arterial stiffness, determination of atherosclerosis prevalence, and assessment of cardiovascular risk becomes clinically important where careful monitoring and correction of risk factors are crucial, especially in patients with longer disease duration.
