Abstract

Cistična fibroza (CF) je autosomno recesivno nasljedna multisistemska bolest koja prvenstveno zahvaća respiratorni, probavni i reproduktivni sustav. Bolest se javlja zbog mutacije gena za transmembranski regulator provodljivosti (CFTR, od engl. cystic fibrosis transmembrane conductance regulator). Cistična fibroza je prvi put opisana 1938. godine na temelju patoanatomskog nalaza kod djece koja su umrla tijekom prve godine života od malapsorpcije i proljeva. Od kraja 80-tih godina terapija i njega za pacijente drastično je unaprijeđena što je utjecalo i na produljenje života pacijenata. Tijekom posljednjih desetljeća postignut je veliki napredak u produljenju bolesnikova životnog vijeka, zahvaljujući uvođenju cijelog niza intervencija u tijeku same bolesti kao što je novorođenački probir, nutritivna potpora, napredak u liječenju respiracijskih smetnji, učinkovitija terapija te transplantacija
pluća. Prema podacima europskog i američkog registra CF bolesnika, danas je prosječni životni vijek bolesnika 44 godine, a u većini zemalja broj odraslih pacijenata veći je od broja djece. Prema podacima iz Europskog registra za cističnu fibrozu, u Hrvatskoj je 45% punoljetnih bolesnika sa cističnom fibrozom. Prema neslužbenim podacima iz nacionalnog registra za 2019. u Hrvatskoj je od CF bolovalo 147 bolesnika. Cistična fibroza danas više nije pedijatrijska bolest, a sve veći broj odraslih bolesnika s cističnom fibrozom donosi sa sobom brojne izazove kako u liječenju tako i praćenju bolesnika. Sa starenjem CF bolesnika samo liječenje postaje kompleksnije i zahtjevnije javljaju se brojne nove komplikacije kako same bolesti tako i dugotrajnog liječenja s kojima se do sada nismo susretali.

Abstract (english)

Cystic fibrosis (CF) is autosomal recessive multisystem disease which primarily affects respiratory, digestive and reproductive system. Disease occurs because of mutation of CFTR gene( cystic fibrosis transmembrane conductance regulator). Cystic fibrosis was first described in 1938. At the time CF patients were dying within the first year of life due to malabsorption and diarrhea. Since late 80s therapy and care for CF patients has drastically improved which prolonged patient survival. Throughout last few decades life expectancy of CF patients has extended due to series of interventions in the course of the disease such as newborn screening, nutritional support, advancements in respiratory therapy and lung transplantation. European and American registry of CF stated that life expectancy of today's patients add up to 44 years. European registry from 2017. stated that Croatia has 91 patient of which 45% are adult, but unofficial information from 2019. registry stated that Croatia had 147 diseased from CF. Due to advancement of medical care, CF ceased to be exclusively pediatric disease,
whereas extended life expectancy of CF patients resulted in greater number of adult patients and new challenges in CF treatment. Owing to older age of patients, their disease gained manifestations and complications which were not yet encountered by physicians.
