Abstract Krvne stanice su eritrociti, trombociti i leukociti. Neutrofili čine najveći dio leukocita, a granulocitni prekursori (prethodnici) u koštanoj srži čine čak 60% svih leukocita. Oni su prve stanice koje odgovaraju na upalu i infekciju, ali isto tako utječu na završetak i cijeljenje povezujući djelovanje urođene i stečene imunosti. ... More posljedice nosi infekcije koje kod tih pacijenata mogu doći bez uobičajenih znakova i simptoma što može stvarati probleme u dijagnostici i pravovremenom prepoznavanju infekcije. Neutropenija može nastati zbog smanjene ili neadekvatne proizvodnje, pretjeranog razaranja ili pomaka neutrofila iz perferije u rezervne bazene te može biti prolazna (akutna) i kronična. Postoje razni uzroci koji mogu dovesti do neutropenije, a mogu se podijeliti u dvije grupe: stečeni i kongenitalni, a stečeni oblici neutropenija znatno su češći. U ovom retrospektivnom radu analizirali smo uzroke, težinu i trajanje neutropenije u djece. Od ukupno 288 ispitanika 281 ispitanik imao je stečenu neutropeniju, a 7 njih kongenitalnu neutropeniju. 37 ispitanika je bilo u skupini blagih neutropenija (ANC > 1,0), njih 76 je bilo u skupini umjerenih neutropenija (ANC = 0,5-1,0), a njih 173 je bilo u skupini teških neutropenija (ANC < 0,5). Kod 227 ispitanika je došlo do ozdravljenja, njih 34 se i dalje prate, a za njih 27 nije poznat ishod neutropenije. Kod ispitanika s poznatim ishodom neutropenije (isključivo ozdravljenje) prosječno je trajanje neutropenije bilo 6,43 mjeseca. Prosječna dob pri postavljanju dijagnoze je bila 19,6 mjeseci. Rezultati su ohrabrujući jer je većina neutropenija benigne naravi, no potrebna su daljnja istraživanja radi boljeg razumijevanja ove bolesti. Less
Abstract (english) Blood cells are erythrocytes, thrombocytes and leukocytes. Neutrophils make the largest portion of leukocytes while granulocyte precursors make up to even 60% of all leukocytes in bone marrow. They are the first responders to inflammation and infection and also regulate resolution of inflammation and healing connecting the responses of innate and adaptive immune systems. More common condition in children and is followed by infections as its main consequence which can occur without common signs and symptoms and that can lead to problems in diagnosing and timely recognition of infection. Neutropenia can happen because of reduced or inadequate production, excessive destruction or shift of the neutrophils from periphery to reserve pools. Neutropenia can be acute or chronic and the causes can be divided in 2 groups: acquired and congenital. Acquired neutropenias are much more common. In this retrospective study we have analyzed causes, severity and duration of neutropenia in children. Out of total of 228 children, 281 of them had acquired neutropenia and 7 of
them had congenital neutropenia. 37 children had mild neutropenia (ANC > 1,0), 76 children had moderate neutropenia (ANC = 0,5-1,0) and 173 children had severe neutropenia (ANC < 0,5). 227 children fully recovered, 34 were still being monitored and 27 had unknown outcome of neutropenia. The average duration of neutropenia in children with known outcome of neutropenia (exclusively recovery) was 6,43 months. The average age of onset was 19,6 months. The results are encouraging because the majority of neutropenias are benign but further researches are needed to better the understanding of this disease. Less
