Abstract

Posljednjih nekoliko desetljeća rekonstrukcija dojke afirmirala se kao nezaobilazna osnova kirurškog liječenja bolesti dojke. Rak dojke, najčešći maligni tumor ženske populacije i vodeći uzrok smrti od malignog oboljenja u žena, najčešća je indikacija za rekonstrukciju dojke. Osim tumorskih promjena, indikaciju za rekonstrukcijom čine i kongenitalne anomalije te stečeni defekti dojke. Razvojem medicine, a tako i kirurških tehnika, dolazi do usavršavanja metoda rekonstrukcije dojke čiji glavni cilj postaje očuvanje anatomskog i strukturnog integriteta tkiva dojke. Rekonstrukcija dojke zahvat je namijenjen ženama u kojih patološki proces narušava cjelovitost tkiva dojke i dovodi do značajnijeg ili manje značajnog morbiditeta. Preoperativna priprema sastoji se od savjetovanja s nadležnim kirurgom te procjene spremnosti kandidata za operativni zahvat rekonstrukcije. S obzirom na vrijeme zahvata, rekonstrukciju je moguće izvesti u istome aktu s mastektomijom, pa se takva rekonstrukcija naziva primarnom. Ukoliko se rekonstrukcija provodi nakon mastektomije te postmastektomičnog liječenja, ona se naziva odgođena ili sekundarna rekonstrukcija dojke. Odabir tehnike rekonstrukcije dojke ovisi o karakteristikama kandidatkinje i spremnosti operatera na izvođenje zahvata. Dvije glavne tehnike jesu rekonstrukcija dojke vlastitim tkivom, poznata kao autologna rekonstrukcija, te rekonstrukcija prostetskim materijalom, odnosno implantatom. Rekonstrukcija se dovršava oblikovanjem kompleksa bradavice i areole. Primjereno odabrana tehnika rekonstrukcije ključna je sastavnica postmastektomičnog rehabilitacijskog procesa.

Abstract (english)

In recent decades, breast reconstruction has been established as an indispensable basis for the surgical treatment of breast diseases. Breast cancer, the most common malignant tumour of the female population and the leading cause of death from malignant diseases, is the most common indication for breast reconstruction. In addition, congenital anomalies and acquired breast deformities are also considered one of the main indications for breast reconstruction. With the development of medicine, as well as surgical techniques, there has been an improvement in breast reconstruction methods whose main goal is preserving the anatomical and structural integrity of breast tissue. Breast reconstruction procedure is intended for women in whom the pathological process disrupts the integrity of breast tissue and leads to more or less significant morbidity. Preoperative preparation consists of consultation with the surgeon and the assessment of the candidate's readiness for reconstructive procedure. Depending on the time of the procedure, the reconstruction can be performed in the same act as the mastectomy, and it's known as the primary reconstruction. If the reconstruction is performed after the mastectomy or postmastectomy radiotherapy, then it's called delayed or secondary reconstruction. The
choice of breast reconstruction technique depends on the characteristics of the candidate and the operator's competence. The two main techniques are breast reconstruction with candidate's own tissue, knows as the autologous reconstruction, and reconstruction with prosthetic material. The reconstruction is completed when nipple-areola complex is restored. Appropriately chosen reconstruction technique is a key component of the post-reconstructive rehabilitation process.
