Abstract

Suvremeni način liječenja i kontrole kroničnih nezaraznih bolesti u koje spada astma nalaže holistički pristup koji sadrži objektivnu i subjektivnu komponentu.tj. uz kliničke pokazatelje uvažava procjenu kvalitete života (HRQOL) koje često nisu u korelaciji. Krajnji cilj u liječenju predstavlja stjecanje kontrole nad bolesti i poboljšanje ukupne kvalitete života.
Astma kao i svaka kronična bolest djeteta mijenja obiteljsku dinamiku i prijašnje modele funkcioniranja i zahtjeva prilagodbu od strane roditelja i djeteta. Stav prema bolesti i reakcije djeteta velikim su dijelom određeni ponašanjem roditelja stoga u proces liječenja astme djeteta neovisno o njegovoj dobi neophodno je aktivno uključiti članove obitelji, posebno roditelje.
Glavni cilj ovog istraživanja je ispitati i usporediti percepciju kvalitete života djece oboljele od astme i njihovih roditelja, a specifični utvrditi kolika je povezanost između objektivnih pokazatelja i rezultata kvalitete života roditelja i djece.
Presječno istraživanje se provodilo u periodu od četiri mjeseca u 2014. godini u Općoj bolnici Virovitica, u Alergološkoj ambulanti Dječjeg odjela. Ispitanici su bili djeca u dobi od 7 do 17 godina koji su dolazili na kontrolni pregled (n=108) te njihovi roditelji (n=97).
U radu su korišteni specifični instrumenti za mjerenje HRQOL u astmi za djecu PAQLQ i roditelje PACQLQ u 3 područja/domene - aktivnosti, emocije i simptoma. Dodatno, roditelji su ispunili upitnik o sociodemografskim obilježjima.
Na temelju dobivenih rezultata, percepcija kvalitete života djece i roditelja značajno se razlikuje, s tim da su djeca iskazala veće zadovoljstvo kvalitetom života od svojih roditelja. Roditelji višeg stupnja obrazovanja iskazali su veće zadovoljstvo kvalitetom života. Komorbiditet i djelomično kontrolirana astma smanjuju kvalitetu života u djece. Djeca s kontroliranom astmom značajno su starija od djece s djelomično kontroliranom astmom, vrijednosti ukupne kvalitete života su bile značajno više u starije djece.

Abstract (english)

A modern treatment and control of chronic non-communicable diseases who include asthma requires a holistic approach and includes objective and subjective opinion of the clinical indicators of HRQOL assessment acknowledges that often do not correlate. The ultimate goal of in the treatment is a acquiring control of the disease and improve the overall quality of life.
Asthma is a a chronic disease, and each child's changing family dynamics and functioning of previous models and requires an adjustment from parents and child. The attitude towards the disease and response of the child are largely determined behavior of the parents therefore in the process of treating asthma child regardless of age, it is necessary to actively involve family members especially their parents.
The main objective of this research to investigate and compared perceptions of the quality of life of children with asthma and their parents, and to determine what the specific association between objective indicators and the results of the quality of life of parents and children.
A cross-sectional study was conducted over a period of four months in the year 2014 at the General Hospital Virovitica, in the infirmary of the Children's Department. Participants were children aged 7-17 years that came to medical examination (n = 108) by their parents (n = 97).
This work were used the specific instruments to measure HRQOL in asthma for children and their parents PAQLQ PACQLQ in three areas / domains - actions, emotions and symptoms. In addition, parents completed a questionnaire on sociodemographic characteristics.
Based on the results, the perception of the quality of life of children and parents is significantly different, with the children expressed greater satisfaction with the quality of life from their parents. The parents higher level of education expressed greater satisfaction with the quality of life. Comorbidity and partly controlled asthma reduces the quality of life in children. The children with controlled asthma were significantly older than children with a partially controlled asthma, the value of the overall quality of life were significantly higher in older kids.
