Abstract

Medikacijske greške nastaju propustom kao nenamjerne radnje u medikacijskom
ciklusu koji uključuje propisivanje , pripremu i primjenu određenog lijeka. Pripadaju
događaju koji se može spriječiti a koji dovodi do neprikladne primjene lijeka i
posljedično tome štetnog djelovanja na pacijenta.
Ukoliko se greške dogode one mogu imati negativne učinke kako na sigurnost i
zdravlje bolesnika tako i na medicinske sestre koje su kontinuirano uključene u direktnu
skrb a i na cjelokupan tim zdravstvenih djelatnika koji skrbe o bolesnicima.
Priprema i podjela lijekova pripada djelu medikacijskog ciklusa koji se odnosi na
djelokrug rada medicinskih sestara u kojem su uočeni brojni čimbenici koji utječu na
njihovu kvalitetu rada a najveći dio njih odnosi se na ometanja i prekide tijekom
primjene lijekova.
Mali broj grešaka dovest će do ozbiljnih ishoda po pacijenta tj. neželjenih događaja.
Takvi događaji bivaju predmet sudskih tužbi, stoga je bolesnikova sigurnost izazov
zdravstvenog sustava današnjice. Kako bi zdravstveni sustavi održali korak s tim
izazovom potrebno je kontinuirano raditi na kulturi sigurnosti a to je moguće ako unutar
radne organizacije postoji razvijen pozitivan stav o prijavljivanju medikacijskih grešaka
ako su se dogodile te sustavni pristup rješavanja istih kako bi se prevenirale buduće
slične greške. Posebno je važno osvijestiti i prijavljivanje onih grešaka koje nisu
uzrokovale štetu pacijentu kako bi se pravovremeno izvršila procjena rizika, a veliku
ulogu u ovom segmentu ima osviještenost zaposlenika o važnosti sigurnosti pacijenata
i kvalitete skrbi.

Abstract (english)

Medication errors are caused by a failure in the medication cycle that involves
prescribing, preparing, and administering a particular drug. They belong to a
preventable event that results in the inappropriate administration of the drug and
consequently a detrimental effect on the patient.
If errors do occur, they can have adverse effects on both the safety and health of
patients and nurses who are continuously involved in direct care and on the entire team
of healthcare professionals who care for patients.
Preparation and distribution of medicines belongs to the part of the medication cycle,
which refers to the scope of work of nurses, which has identified a number of factors
that affect their quality of work and most of them related to interruptions and
interruptions during drug administration .
A small number of errors will lead to serious patient outcomes, ie adverse events,
which are the subject of lawsuits, so patient safety is a challenge to the health care
system today, and continuous work on a safety culture is required, which is possible if
a positive reporting attitude is developed within the work organization. medication
errors, if any, and a systematic approach to address them to prevent future similar
errors. It is especially important to be aware of the reporting of those errors that did not
cause harm to the patient in order to conduct a timely risk assessment and a major
role in this segment is the awareness of employees about the importance of patient
