Abstract

Sveobuhvatni probir novorođenčadi na oštećenje sluha doveo je do povećane detekcije oštećenja sluha kod vrlo male djece. Prema SZO u svijetu postoji preko 32 miliona djece koja pate od ovoga problema. Kohlearna implantacija operacijski je postupak ugradnje umjetne pužnice teško nagluhim ili u potpunosti gluhim osobama. Ovom intervencijom može se pomoći djeci da od najranije dobi žive kao i njihovi vršnjaci. Postavlja se pitanje najprikladnije dobi za ovaj zahvat u slučaju kongenitalno oštećenog sluha. Uz poteškoće prilikom postavljanja točne dijagnoze u ovoj dobnoj skupini te rizike anestezioloških mjera i samog kirurškog postupka, istraživanja su pokazala da je korist izvođenja kohlearne implantacije, kod djece dobi do 2 godine, puno veća od rizika. Tehnologija pa tako i kirurgija i anesteziologija napreduju svakog dana te tako omogućavaju revolucionarne stvari kao što je vraćanje ireverzibilnog gubitka sluha. Iz tog razloga, ovom se zahvatu pristupa kao najboljoj opciji pri optimiziranju rasta i razvoja prelingvalno gluhih pacijenata.

Abstract (english)

Widespread universal newborn hearing screening has led to increased identification of infants with hearing loss worldwide. According to the WHO there are 32 millions of children affected by this problem. Cochlear implantation is a procedure which restores function of the cochlea to severely deaf people. This intervention can help very young children to become independent and active members of society. The thesis of this paper is to prove weather the age under two years is appropriate for this action in congenitally deaf children. The biggest considerations are diagnostic methods needed to assess the severity of hearing loss in this specific populations as well as surgical and anesthesiologic risks. However, the research has shown that children who get implanted before second birthday can live almost a normal life so this procedure is thought considered as therapy of choice. Some risks are recommended to be taken since medical technology along with surgery and anesthesiology are moving forward and paving the way to the healthier future.
