Abstract

Spondiloartritisi heterogena su skupina međusobno povezanih upalnih reumatskih bolesti karakterizirana zajedničkim genetičkim, kliničkim i radiološkim obilježjima. U nju spadaju ankilozantni spondilitis, kao glavni predstavnik skupine, psorijatični artritis, enteropatski artritis, reaktivni artritis i kronični juvenilni idiopatski artritis. U posljednja dva desetljeća došlo je do brojnih novih spoznaja o spondiloartritisima, novih klasifikacija, boljeg razumijevanja genetičkih i patofizioloških... More mehanizama i razvoja novih dijagnostičkih i terapijskih opcija. Tjelesna aktivnost i terapijske vježbe tijekom cijelog tog razdoblja, a i danas, ostale su jedan od glavnih terapijskih modaliteta u liječenju spondiloartritisa. Glavni ciljevi kineziterapije u liječenju SpA su postizanje i očuvanje maksimalne potencijalne pokretljivosti, prevencija posturalnih deformacija, poboljšanje mišićne snage i kondicije, povećanje gustoće kostiju i smanjenje bola. U liječenju SpA svi tipovi terapijskih vježbi imaju svoju ulogu, a njihova primjena može biti individualna ili grupna. Istovremeno se uz njih mogu primjenjivati i drugi modaliteti fizikalne terapije, poput hidroterapije i primjene topline, kao i farmakološka sredstva, od kojih najvažniji učinak imaju inhibitori TNF-α. U više studija dokazan je učinak terapijskih vježbi u poboljšanju tjelesne funkcije, smanjenju boli i aktivnosti bolesti, dok je učinak na spinalnu mobilnost i umor još uvijek upitan. Još uvijek ne postoje specifične preporuke i programi terapijskih vježbi u liječenju SpA i potrebna su kvalitetna daljnja istraživanja u tu svrhu. Less

Abstract (english)

Spondyloarthritis is a heterogeneous group of internally connected inflammatory rheumatic diseases, characterized by mutual genetic, clinical, and radiographic features. It consists of ankylosing spondylitis, the group's main representative, psoriatic arthritis, arthritis related to inflammatory bowel disease, reactive arthritis, and chronic juvenile idiopathic arthritis. More a better understanding of genetic and pathophysiological mechanisms, and the development of new diagnostic and therapy
resources. Physical activity and therapeutical exercise have, throughout this period, remained as one of the main therapeutic modalities in the treatment of spondyloarthritis. The main goals of kinesiotherapy in the treatment of SpA are achieving and preserving maximal potential mobility, prevention of postural deformities, improving muscle strength and condition, increasing bone density, and pain reduction. All types of therapeutic exercises are utilized in the treatment of SpA, and they can be applied individually or in groups. Simultaneously, other physical therapy modalities, such as hydrotherapy and heat, can be practiced, as well as pharmacological treatment, of which TNF-α inhibitors are the most important. The effects of therapeutic exercises on improving physical function, pain and disease activity reduction have been demonstrated in multiple studies, while the effect on spinal mobility and fatigue is still questionable.
