Abstract Redukcijska kirurgija dojki operativni je postupak koji se izvodi da bi se smanjio volumen tkiva dojke, očuvao kompleks areole i bradavice, oblikovao kožni omotač i postigao estetski zadovoljavajući oblik dojke. Zahvat se izvodi iz funkcionalnih i estetskih razloga. Redukcijska mamoplastika indicirana je u stanjima hipertrofije i ptoze dojke, makromastije ili kongenitalne asimetrije dojki. Simptomi koji se javljaju uslijed prekomjerne veličine dojki uključuju bolove u vratu, odnosno u kralješnici, bolove u dojkama, glavobolju, osip, maceraciju i iritacije koje se javljaju na koži ispod dojke. Navedeno može negativno utjecati na psihičko stanje pacijentice i kvalitetu svakodnevnog života. Potrebna je pravilna i detaljna prijeoperativna priprema i označavanje položaja incizije da bi se postigao estetski što prihvatljiviji rezultat. Uz sve brži razvoj novih kirurških tehnika, u načelu postoje četiri osnovne tehnike: redukcijska mamoplastika obrnutog slova T, vertikalna redukcijska kirurgija, liposukcijska metoda i SPAIR mamoplastika. Prilikom korištenja pojedine tehnike nužno je poznavati njene prednosti i nedostatke. Operativni zahvat redukcijske mamoplastike nosi svoje rizike i komplikacije. Neznatne komplikacije u ranom postoperativnom periodu uključuju pojavu hematoma, seroma, celulitis, infekciju rane i odgođeno cijeljenje rane. Značajnije komplikacije koje često zahtijevaju kiruršku intervenciju uključuju nekrozu režnja, nekrozu kompleksa bradavice i areole i dehiscijenciju rane. Važno je pravilno odabrati pacijenta prema indikacijama, pravilno odabrati kiruršku tehniku i upoznati ih o mogućim komplikacijama zahvata kako bi se smanjila mogućnost nepovoljnog ishoda.
Abstract (english) Breast reduction surgery is a surgical procedure done to reduce breast tissue volume, maintain nipple-areola viability and achieve an aesthetically pleasing shape. The procedure is done for functional and aesthetic reasons. Reduction mammoplasty is indicated when there is breast hypertrophy and breast ptosis, macromastia or congenital breast asymmetry. Symptoms occurring due to excessively large breasts include neck pain, namely pain in the spine, breast pain, headache, rash, maceration and irritation found on the skin beneath a breast. The symptoms can have a negative impact on the patient’s mental state and everyday life quality. To achieve an aesthetically pleasing result, regular and detailed preoperative care, as well as incision site marking, is needed. Together with rapid surgical techniques development, there are four basic techniques: Inverted T Pattern Inferior Pedicle Breast Reduction, Vertical Breast Reduction, Liposuction Breast Reduction and SPAIR Mammoplasty. While using a certain technique it is necessary to know its advantages and disadvantages. The reduction mammoplasty procedure has its risks and complications. Minor complications in the early postoperative period include hematoma, seroma, cellulitis, wound infection and delayed wound healing. Major complications that often require surgical intervention include flap necrosis, nipple-areolar necrosis and wound dehiscence. It is important to choose the patient properly according to the indications, to choose the surgical technique properly and to make the patient familiar with possible complications in order to reduce the possibility of an unfavourable outcome.
