Abstract

Eritemski lupus (LE) je kronična, autoimunosna bolest koja uključuje širok spektar simptoma. Podijeljena je na sustavni oblik – SLE i kožni oblik – CLE koji se mogu pojaviti zajedno ili odvojeno.
Kuhn i suradnici su 2003. godine razvili modificirani klasifikacijski sustav koji se naziva „Düsseldorfska klasifikacija kožnog eritemskog lupusa 2003“. Prema toj klasifikaciji CLE se dijeli na akutni kožni eritemski lupus (ACLE), subakutni CLE (SCLE), kronični CLE (CCLE) kod kojeg je klasični diskoidni LE (DLE) najčešći oblik, lupus eritematosus tumidus (LET) kao intermitentni podtip CLE-a (ICLE) i bulozne kožne lezije povezane sa LE (BLE).
Svi podtipovi kožnih oblika eritemskog lupusa su definirani specifičnim kliničkim simptomima, prosječnim trajanjem simptoma te histološkim i serološkim nalazima, međutim vrlo često se klinička obilježja preklapaju. Pacijenti sa kožnim oblicima eritemskog lupusa imaju dobro definirane kožne lezije, često na mjestima izloženima suncu. Bolest se uglavnom očituje kroničnim tijekom i relapsima izazvanima izlaganjem sunčevu svjetlu. Vrlo je važno potvrditi dijagnozu CLE-a histopatološki biopsijom jer postoji nekoliko diferencijalnih dijagnoza te redovito kontrolirati bolesnika zbog mogućnosti prelaska u sistemni oblik bolesti. Liječenje CLE-a uključuje zaštitu od sunca, lokalnu terapiju i sistemsku terapiju.

Abstract (english)

Lupus Erythematosus (LE) is a chronic, autoimmune disease that includes a broad spectrum of symptoms. It is divided into one systemic form – SLE and one cutaneous form – CLE. They can occur both together and separately.
In 2003 Kuhn and al. developed a modified classification system named „Düsseldorf Classification of Cutaneous Lupus Erythematosus 2003“. The classification system divided CLE to acute CLE (ACLE), subacute CLE (SCLE), chronic CLE (CCLE) where discoid LE (DLE) is the most common form, lupus erythematosus tumidus (LET) as the intermittent subtype of CLE (ICLE) and bullous lesions in lupus erythematosus (BLE).
All subsets are defined by clinical symptoms, average duration of symptoms and histological and serological findings, although, the subtypes can have overlapping clinical features. CLE patients display well-defined skin lesions, often in sun-exposed areas. The disease often has a chronic and relapsing course that can be induced or aggravated by UV light. It is important to confirm a CLE diagnosis histopathologically by a biopsy and in that there are several differential diagnoses and regular follow-up is important because progression to SLE can occur. CLE treatment combines sun protection, topical therapies, and systemic agents.
