Abstract

Moždani udar drugi je vodeći uzrok smrti u svijetu te treći uzrok smrti i nesposobnosti
zajedno. Razlikujemo ishemijski i hemoragijski MU te subarahnoidalno krvarenje.
Ishemijski moždani udar daleko je češći od hemoragijskog i subarahnoidalnog krvarenja
te se ovaj rad prvenstveno bavi ishemijskim moždanim udarom. Najčešći mehanizmi nastanka
ishemijskog moždanog udara su makroangiopatija, mikroangiopatija i kardioembolija, koji čine
preko 65% uzroka ishemijskog moždanog udara. Optimalno liječenje bolesnika sa sumnjom na
moždani udar sastoji se od adekvatnog i pravovremenog prehospitalnog zbrinjavanja te bolničkog
zbrinjavanja bolesnika. Liječenje ishemijskog moždanog udara obuhvaća primarnu prevenciju,
akutno liječenje te sekundarnu prevenciju. Osnovni cilj liječenja akutnog ishemijskog moždanog
udara su rekanalizacija i reperfuzija. Danas dostupne metode rekanalizacije obuhvaćaju
intravensku trombolizu i mehaničku trombektomiju. Intravenska tromboliza je terapijski postupak
primjene rekombinantnog tkivnog aktivatora plazminogena s ciljem razgradnje tromba ili
embolusa koji je uzrokovao okluziju krvne žile. Najčešće primjenjivani aktivator plazminogena je
alteplaza. Glavni nedostatak intravenske trombolize je uzak vremenski prozor u kojem se alteplaza
može primijeniti, a iznosi 4.5 sata od pojave simptoma. Velik problem u određivanju početka
simptoma predstavljaju ishemijski moždani udari koji se javljaju u snu. Zbog uskog vremenskog
prozora, međunarodne smjernice propisuju brojne relativne i apsolutne kontraindikacije za
primjenu intravenske trombolize, a s ciljem očuvanja dobrog sigurnosnog profila. Najvažnije
komplikacije primjene alteplaze su intracerebralno krvarenje i angioedem. Mehanička
trombektomija standard je u liječenju bolesnika s okluzijom velikih krvnih žila prednje cirkulacije.
Vremenski prozor mehaničke trombektomije je nešto veći nego je to slučaj kod intravenske
trombolize te se može sigurno primijeniti kod odabranih bolesnika čak do 24 sata nakon pojave
simptoma moždanog udara. Uspješnost rekanalizacije pomoću mehaničke trombektomije
procjenjuje se pomoću TICI i eTICI skale. Učestalost najopasnije komplikacije, simptomatskog
intracerebralnog krvarenja, rjeđa je kod primjene mehaničke trombektomije nego kod primjene
intravenske trombolize. U praćenju funkcionalnog oporavka bolesnika nakon preboljelog
moždanog udara koristimo se mRS i NIHSS ljestvicama. Iako je rekanalizacijsko liječenje
revolucioniziralo zbrinjavanje bolesnika s ishemijskim moždanim udarom, ono se i dalje ne
primjenjuje u dovoljnom opsegu te u sljedećim godinama očekujemo njen daljnji napredak.

Abstract (english)

Stroke is the second leading cause of death in the world and the third leading cause of death and
disability combined. We distinguish three types of stroke – ischemic stroke, hemorrhagic stroke
and subarachnoid hemorrhage. Ischemic stroke is far more common than hemorrhagic stroke and
subarachnoid hemorrhage, and this paper primarily deals with ischemic stroke. The most
common mechanisms of ischemic stroke are macroangiopathy, microangiopathy and
cardioembolism, which account for over 65% of ischemic stroke causes. Optimal treatment of
patients with suspected stroke consists of adequate and timely pre-hospital care and inpatient
care. Treatment of ischemic stroke includes primary prevention, acute treatment, and secondary
prevention. The main goals of treatment of acute ischemic stroke are recanalization and
reperfusion. Today, recanalization methods include intravenous thrombolysis and mechanical
thrombectomy. Intravenous thrombolysis is a therapeutic procedure using a recombinant tissue
plasminogen activator to break down a thrombus or embolus that has caused the occlusion of a
blood vessel. The most commonly used plasminogen activator is alteplase. The main disadvantage
of intravenous thrombolysis is the narrow time window in which alteplase can be administered,
which is 4.5 hours from the onset of symptoms. Ischemic strokes, which occur in sleep, are a major
problem in determining the onset of symptoms. Due to the narrow time window, international
guidelines prescribe a number of relative and absolute contraindications for the use of intravenous
thrombolysis, intending to maintain a good safety profile. The most important complications of
alteplase are intracerebral hemorrhage and angioedema. Mechanical thrombectomy is the
standard in the treatment of patients with large vessel occlusion in the anterior circulation. The
time window of mechanical thrombectomy is slightly larger than that of intravenous thrombolysis
and can be safely applied in selected patients up to 24 hours after the onset of symptoms. The
success of recanalization by mechanical thrombectomy is assessed using the TICI and eTICI
scales. The frequency of the most dangerous complication, symptomatic intracerebral
hemorrhage, is less common with mechanical thrombectomy than with intravenous thrombolysis.
We use mRS and NIHSS scales to monitor the functional recovery of patients after a stroke.
Although recanalization treatment has revolutionized the care of patients with ischemic stroke, it
is still not applied to a sufficient extent and we expect further progress in the coming years.
