Učinkovitost biološke terapije u liječenju perianalne Crohnove bolesti

Abstract

Crohnova bolest je upalna bolest crijeva koja može zahvatit bilo koji dio probavnog sustava. Perianalna bolest je jedna od komplikacija Crohnove bolesti koja se očituje nastankom perianalnih apscesa i fistula. U liječenju perianalnog oblika Crohnove bolesti danas se najčešće upotrebljava biološka terapija. U ovom istraživanju sakupili smo podatke o bolesnicima s dijagnozom perianalne Crohnove bolesti koji su se liječili biološkom terapijom u Dnevnoj bolnici Zavoda za gastroenterologiju i hepatologiju Klinike za unutarnje bolesti KBC Zagreb u periodu od 1.1.2017. do 1.4. 2021. te usporedili učinkovitosti terapija u postizanju kliničke remisije. Ukupno je uključeno 63 bolesnika. Infliximab je koristilo 32, adalimumab 17, vedolizumab 2 te ustekinumab 12 bolesnika. Učinkovitosti terapija iznose za infliximab ukupno 71% (p=0.008), od toga kada je korišten samo infliximab 50% (p=0,773), a kada je korišten infliximab sa imunomodulatorom 76.9% (p=0.004). Za adalimumab ukupno 64.7% (p=0,374). Učinkovitost ustekinumaba ukupno je iznosila 8.3% (p<0.001). Zaključno, zbog nepravilne raspodijele pacijenata među terapijom, potvrditi se može samo učinkovitost infliximaba, dok drugi biološki lijekovi moraju biti podvrgnuti daljnjim istraživanjima na većem uzorku.

Abstract (english)

Crohn's disease is an inflammatory bowel disease that can affect any part of the gut. Perianal disease is one of the complications of Crohn's disease, manifested by the onset of perianal abscesses and fistula. In the treatment of the perianal form of Crohn's disease, biological therapy is most often used today. In this study, we collected data on patients diagnosed with perianal Crohn's disease who were treated with biological therapy in the Day hospital of the Department of Gastroenterology and Hepatology of the Clinic for Internal Diseases of the KBC Zagreb in the period from 1.1.2017 to 1.4.2021. and compared the efficacy of biologic therapies in achieving clinical remission. A total of 63 patients participated. Infliximab was used by 32, adalimumab 17, vedolizumab 2 and ustekinumab 12 patients. The effectiveness’s of therapies are amounted to a total of 71% for infliximab (p=0.008), infliximab monotherapy 50% (p=0.773), infliximab combined with immunomodulator 76.9% (p=0.004). For adalimumab, a total of 64.7% (p=0.374). The efficiency of ustekinumab was 8.3% (p<0.001). In conclusion, due to the unequal distribution of patients among the therapy, only the effectiveness of infliximab can be established, while other biologic drugs must be subject to further research on a larger sample.
