Abstract

Starenje kože je složen, degenerativni proces koji se dijeli na intrinzično starenje i fotostarenje. Rezultat tog procesa su brojne kozmetski nepoželjne promjene na koži kao što su suhoća, gubitak elasticiteta, dispigmentacije, teleangiektazije i bore. Liječenje fotostarenja uključuje različite farmakološke i kirurške postupke kako bi se uklonila postojeća oštećenja. Poboljšanje izgleda ostarjele kože zahtjeva promjene u dermalnom kolagenu. Izbjegavanje izlaganja sunčevom zračenju te primjena zaštitnih krema i odjeće, predstavljaju prvu liniju obrane od fotostarenja. Kozmeceutici su sredstva koja sadrže biološki aktivne sastojke kao što su peptidi, antioksidansi te biljni ekstrakti, ali samo nekoliko objavljenih studija opisuje njihovu učinkovitost u liječenju starenja. Topikalni retinoidi su derivati vitamina A koji imaju dokazan antiage učinak. U terapiji starenja kože primjenjuju se retinoidi tazaroten i tretionin. Laseri i terapija svjetlom uključuju ablativne i neablativne metode liječenja, a njihov mehanizam djelovanja temelji se na principu selektivne fototermolize. Jedna od metoda liječenja starenja kože je i primjena botulinum toksina. Botulinum toksin je neuromodulator koji uzrokuje privremenu relaksaciju podležeće muskulature te stvara efekt pomlađivanja kože. Najčešće se primjenjuje botulinum toksin tip A. Dermalni fileri vraćaju mladolik izgled licu tako što nadomještaju izgubljen tkivni volumen te popunjavaju i brišu bore. Važno je prije primjene određene metode liječenja informirati pacijenta o očekivanim rezultatima, vremenu oporavka te mogućim rizicima postupka.

Abstract (english)

Skin aging is a complex degenerative process which can be divided into two basic processes: normal aging and photoaging. It produces many cosmetically undesirable changes in skin. Photoaged skin is characterized by dryness, laxity, dyspigmentation, teleangiectasia, deep creases and rhytides. Medical treatment of photoaging includes medications and procedures to reverse existing damage. Improvement in rhytides and skin laxity require the induction of changes in dermal collagen. Sun avoidance and the use of sunscreen and protective clothing is very important and it is the first line of defense against photoaging. Cosmeceuticals are agents that contain biologically active ingredients such as peptides, antioxidants and botanicals, but there are only a few published studies on their efficacy. Topical retinoids are derivates of vitamin A that have antiaging properties. Tazaroten and tretinoin are available for the skin aging treatment. Lasers and light treatment include ablative and nonablative laser system. They work through mechanism called selective photothermolysis. Botulinum toxin is a neuromodulator that temporarily weakens muscles and gives the appearance of rejuvenation. Botulinum toxin type A injections are the most popular product. Soft tissue augmentation or ‘‘fillers’’ reduce contour defects through the replacement of tissue volume lost due to aging. Patients should be informed about the expected outcomes, anticipated recovery period and treatment risks.
