Abstract

Sestrinska skrb za oboljele od multiplog mijeloma predstavlja izazov sestrinstvu. Bolest je progresivna i maligna. Početak joj je obično nespecifičan s nizom simptoma koji se mogu javiti i u drugim bolestima. Pri postavljanju dijagnoze koriste se specifični postupci, uključujući vađenje krvi, punkciju ili biopsiju koštane srži te slikovne metode u kojima sudjeluju i medicinske sestre. Liječenje ovisi o brojnim čimbenicima. U terapiji najčešće se primjenjuje kemoterapija za koju medicinske sestre trebaju biti odgovarajuće educirane, kako za samu provedbu tako i za praćenje općeg stanja bolesnika. U liječenju se još koristi ciljana protutmorska terapija, steroidi, zračenje i bifosfonati. Terapijska metoda u kojoj je vrlo važna aktivna uloga medicinskih sestara, a koja se često koristi u liječenju ove bolesti je transplantacija autolognih matičnih krvotvornih stanica. Multipli mijelom je neizlječiva bolest pa bolesnici redovito dobivaju nekoliko linija terapije čiji izbor i redoslijed ovise o stanju bolesnika, značajkama bolesti i odgovoru na ranije liječenje. Medicinske sestre dobrom edukacijom bolesnika i njegove obitelji pomažu u savladavanju psihičkih i fizičkih promjena koje prate ovu bolest i liječenje. Bolesnik veći dio vremena liječenja provodi u svom domu te je provođenje kontinuirane sestrinske skrbi od sekundarne ili tercijarne zdravstvene zaštite preko primarne zdravstvene zaštite važan proces zdravstvene njege bolesnika.

Abstract (english)

Nursing patients with multiple myeloma is a challenge. The disease is progressive and malignant; its onset is usually non-specific with a number of symptoms that can occur in other diseases. Diagnostics require specific procedures including venepuncture, bone marrow biopsy and imaging methods in which nurses participate. Choice of treatment depends on multiple factors; chemotherapy is used most frequently. Nurses need to be adequately educated for both implementation of chemotherapy and monitoring the general condition of the patient. Other treatment options include targeted therapy, steroids, radiotherapy and biphosphonates. Autologous stem cell transplantation is a therapeutic method frequently used in this disease in which active participation of nurses is of utmost importance. Since multiple myeloma is an incurable disease, patients regularly receive multiple line soft treatment; selection of order in which they are applied depend on patient's condition, disease characteristics and response to previous treatment. Nurses can aid patients and their families in coping with psychological and physical changes accompanying the disease and its therapy by providing proper education. Patients spend most of the time during treatment at home, therefore implementation of continuous nursing care, from secondary or tertiary up to primary health care, is important for the quality of the nursing process.
