Abstract

Katarakta ili siva mrena zamućenje je prirodno prozirne očne leće. Postupnim zamućenjem leće ograničen je prolazak svjetlosnih zraka na mrežnicu što uzrokuje slabljenje vida, a u konačnici može rezultirati sljepoćom. Jedina metoda liječenja je kirurška. Tehnika koja se najčešće primjenjuje, ultrazvučna fakoemulzifikacija, u suvremenoj kirurgiji katarakte unaprjeđena je primjenom femtosecond lasera. S napretkom tehnologije i usavršavanjem kirurških tehnika uspjeh zahvata je iznimno visok. Međutim, i dalje nije potpuno otklonjen rizik od razvoja komplikacija. Komplikacije koje se razvijaju na stražnjem očnom segmentu, premda rijetke, prijete gubitku vida. Od cistoidnog makularnog edema kao najčešće, odvajanja mrežnice, suprakoroidalnog krvarenja pa do endoftalmitisa, sve su komplikacije teško liječiva stanja. Stoga, potrebno je u predoperativnoj obradi procijeniti individualni rizik za svakog bolesnika te uložiti značajne napore u prevenciju. Kad se komplikacije ipak razviju, promptno prepoznavanje, dijagnostika i učinkovito liječenje ključno je za konačan povoljan ishod.

Abstract (english)

A cataract is a clouding of the naturally transparent lens of the eye. The gradual clouding of the lens limits the passage of light rays to the retina, which causes weakening of vision, and ultimately can result in blindness. The only method of treatment is surgery. The most commonly used technique, ultrasound phacoemulsification, in modern cataract surgery has been improved by the use of a femtosecond laser. With the advancement of technology and the improvement of surgical techniques, the success of the procedure is extremely high. However, the risk of developing complications is still not completely eliminated. Complications that develop in the posterior segment of the eye, although rare, they are sight-threatening. From cystoid macular edema as the most common, retinal detachment, suprachoroidal hemorrhage to endophthalmitis, all complications are difficult to treat conditions. Therefore, in the preoperative treatment, it is necessary to assess the individual risk for each patient and invest significant efforts in prevention. When complications develop, prompt recognition, diagnosis, and effective treatment are key to a ultimately good outcome.
