Abstract

Literatura koja se bavi kirurgijom kompleksnih ventralnih hernija trbušne stijenke u hrvatskoj medicini je slabo zastupljena te su nova saznanja nužno potrebna kirurzima kako bi uspješnije liječili ovaj složen skup kirurških bolesti. Kompleksne ventralne hernije trbušne stijenke se mogu definirati po veličini i lokaciji, kontaminaciji i stanju mekih česti, anamnezi pacijenta i rizičnim faktorima te po kliničkim scenarijima. Postoji više kirurških tehnika koje se koriste u liječenju: otvorenim pristupom, laparoskopski, robotski ili kombinacijom navedenih. Ključan je multidisciplinarni rad zbog heterogenosti slučajeva. Kod kontaminacije se preporučaju spororesorptivne biološke mrežice, dok za liječenje inficiranih mrežica i dalje nema službenih smjernica i vodiča. U ovom radu obrađena su recentna istraživanja o terapiji i smjernicama primjenjivim u budućnosti u liječenju kompleksnih ventralnih hernija trbušne stijenke u kojoj robotska kirurgija zauzima sve veće mjesto.

Abstract (english)

The literature dealing with the surgery of complex ventral abdominal wall hernias is poorly represented in Croatian medicine, and new knowledge is urgently needed by surgeons in order to more successfully treat this complex set of surgical diseases. Complex ventral abdominal wall hernias can be defined by size and location, soft tissue contamination and condition, patient history and risk factors, and clinical scenarios. There are several surgical techniques used in treatment: open access, laparoscopic, robotic or a combination of the above. Multidisciplinary work is crucial due to the heterogeneity of cases. Slowresorbable biological meshes are recommended for contamination, while there are still no official recommendation and guidelines for the treatment of infected meshes. This paper discusses recent research on therapies and guidelines applicable in the future in the treatment of complex ventral abdominal wall hernias in which robotics is more and more finding its way into surgery.
