Abstract

Malnutricija je stanje koje zahvaća velik broj hospitalizirane djece, a u većine je prisutna već u trenutku prijema u bolnicu. Kod kronično bolesnog djeteta incidencija malnutricije varira između 25-70%. Velik rizik za razvoj malnutricije imaju djeca mlađa od pet godina te oni koji boluju od različitih poremećaja i bolesti gastrointestinalnog sustava. Usprkos tome što postoje određene metode probira za otkrivanje pothranjenosti, prilikom prijema u bolnicu još uvijek se previdi velik broj djece koja su pothranjena ili imaju rizik za razvoj pothranjenosti. Važno je pomoću adekvatne metode procijeniti potrebe pojedinca i na temelju toga uvesti nutritivnu potporu oralnim hiperkalorijskim pripravcima ili enteralnom prehranom ako za nju postoje indikacije. Ako pak dijete zbog poremećaja gastrointestinalnog sustava ili netolerancije drugih oblika prehrane ne može primati enteralnu prehranu, javlja se potreba za parenteralnom prehranom koju je uvijek dobro kombinirati s enteralnom, čak i u minimalnim količinama. Nutritivna potpora kronično bolesnog djeteta zahtjeva multidisciplinarni pristup i timsko zbrinjavanje djeteta.

Abstract (english)

Malnutrition is a condition that is observed among large number of hospitalized children and most of them suffer from it already when arriving in the hospital. Chronically ill child has 25-70% incidence of malnutrition. Children younger than five years and those suffering from various disorders and illnesses of gastrointestinal system, have large risk for malnutrition development. Despite the screening tools for discovering malnutrition, at the admission to the hospital large number of children that are, or have risk for becoming undernourished, are not observed. It is important to estimate the needs of individuals by adequate methods and induce oral hypercaloric formulations or enteral nutrition if necessary. If child has other gastrointestinal disorders or intolerance and cannot take enteral nutrition, there is a need for parenteral nutrition that can be combined with enteral, even in minimal quantities. Nutritive support of chronically ill child demands multidisciplinary approach and team care for the child.
