Abstract

Vremenska era otkrića antibiotika podigla je kvalitetu ljudskog života i zaustavila dotad neke smrtonosne bolesti. S druge strane, povećanje primjene antibiotika donijelo je i rezistentnost mikroorganizama na antibiotike. Antimikrobna rezistencija (AMR) unatoč napretku u dijagnostičkim i terapijskim protokolima, predstavlja globalnu prijetnju javnom zdravstvu te veliki izazov u borbi između ljudske domišljatosti i prilagodbe samih mikroorganizama. I Republika Hrvatska u sklopu „Odbora za praćenje rezistencije bakterija na antibiotike“ nadzire rezistentnost mikroorganizama na antibiotike s posebnim naglaskom na ESKAPE multirezistentne bakterije.
Multirezistentni organizmi se šire manjkom kvalitetne edukacije, neadekvatnom i nepotrebnom uporabom antibiotika, manjku higijene i prenapučenošću. Za prevenciju i kontrolu multirezistentnih mikroorganizama u zdravstvenim ustanovama važna je djelotvorna implementacija programa upravljanja primjene antibiotika kako bi se spriječila pojava rezistentnih mikroorganizama i primjena mjera kontrole infekcija uz osnovne mjere zaštite.
Cilj rada je utvrditi učestalost višestruko otpornih bakterija iz mikrobioloških uzoraka na Klinici za bolesti srca i krvnih žila Kliničkog bolničkog centra (KBC) Sestre milosrdnice te prikazati najčešće uzorke iz kojih je izolirana višestruko otporna bakterija u razdoblju od 08/2020 do 11/2021 iz uzoraka hemokultura, aspirata traheje i bronha, brisa nosa, spojnice oka i rana te urina i brisa rektuma.
Rezultati pokazuju kako multirezistentni mikroorganizmi čine 12% ukupnog broja uzoraka u Klinici za bolesti srca i krvnih žila KBC-a Sestre milosrdnice (N=1221), od čega je 33% A. baumannii complex (N=353) izoliran iz uzorka urinokulture.

Abstract (english)

The discovery era of antibiotics raised the quality of human life and stopped some deadly diseases. On the other hand, the increase in antibiotic consumption has also brought resistance of microorganisms to antibiotics. Antimicrobial resistance (AMR), despite advantages in diagnostic and therapy, poses a global threat to public health and a major challenge in the fight between human ingenuity and the adaptation of microorganisms themselves. The Republic of Croatia, within the "Committee for Antibiotic Resistance Surveillance in Croatia", also monitors the resistance of microorganisms to antibiotics, with special emphasis on ESKAPE multidrug-resistant bacteria.
Multidrug-resistant organisms are spread by lack of quality education, inadequate and unnecessary use of antibiotics, lack of hygiene, and overcrowding. For the prevention and control of multidrug-resistant microorganisms in health care institutions, the effective implementation of antibiotic management programs is important in order to prevent the emergence of resistant microorganisms and the application of infection control measures with basic protection measures.
The aim of this study was to determine the frequency of multidrug-resistant bacteria from microbiological samples at the Clinic for Cardiovascular Diseases University Hospital Center Sestre milosrdnice and to show the most common samples from which multidrug-resistant bacteria were isolated in the period 08/2020 to 11/2021 from haemoculture, tracheal and bronchial aspirates, nasal eye, wound, urine, and rectal swabs.
The results show that multidrug-resistant microorganisms make up 12% of the total number of samples in the Clinic for Cardiovascular Diseases of UHC Sestre milosrdnice (N = 1221), of which 33% A. baumannii complex (N = 353) was isolated from a urine culture sample.
