Ona se može javiti zajedno s drugim kroničnim komplikacijama istovremeno, a češća je kod bolesnika s lošom kontrolom glikemije tijekom liječenja. Patofiziologija ove komplikacije iznimno je složena te je još uvijek velikim dijelom nerazjašnjena, a uključuje interakciju brojnih endogenih i egzogenih čimbenika. Dijabetička gastroenteropatija može zahvatiti čitavu dužinu gastrointestinalnog trakta, a simptomi najčešće uključuju disfagiju, dispepsiju, abdominalnu bol, nadutost, dijareju, konstipaciju i fekalnu inkontinenciju. Važno je naglasiti da su funkcionalni poremećaji gastrointestinalnog sustava česti u općoj populaciji, ali i populaciji dijabetičara (oko četvrtina svih oboljelih), te se stoga oni često ne mogu jednostavno razabrati od dijabetičke gastroenteropatije samo na temelju simptomatologije ili pozitivne osobne anamneze za šećernu bolest. S obzirom na rastuću prevalenciju šećerne bolesti, ova komplikacija će poprimati sve veći značaj zbog iznimnog utjecaja na kvalitetu života bolesnika s razvijenom komplikacijom. Iako je ona prvi puta opisana u literaturi prije gotovo pola stoljeća, dijagnoza i liječenje ovog patološkog stanja i dalje predstavljaju veliki izazov za modernu medicinu. U skladu s time, dijabetička gastroenteropatija je predmet velikog broja znanstvenih istraživanja s konačnim ciljem unaprjeđenja kvalitete života svih bolesnika koji su razvili ovu kroničnu komplikaciju. Kako bi se postigao standard u liječenju, važno je kvalitetno educirati liječnike s ciljem što ranijeg prepoznavanja, uz naglasak na multidiciplinarni pristup u pružanju zdravstvene skrbi ovih bolesnika.
Abstract (english) Diabetic gastroenteropathy is a chronic complication of diabetes mellitus resulting from autonomic neuropathy, most commonly seen in long-term patients with type 1 and type 2 diabetes. It can occur concurrently with other chronic complications and is more frequent in patients with poor glycemic control. The pathophysiology of this complication is extremely complex and remains largely unclear, involving the interaction of numerous endogenous and exogenous factors. Diabetic gastroenteropathy can affect the entire length of the gastrointestinal tract, with symptoms often including dysphagia, dyspepsia, abdominal pain, bloating, diarrhea, constipation, and fecal incontinence. It is important to note that functional gastrointestinal disorders are common in both the general population and among diabetics (affecting about one-quarter of all patients), making it difficult to distinguish diabetic gastroenteropathy based solely on symptoms or a positive medical history of diabetes. Given the increasing prevalence of diabetes, this complication is expected to become more significant due to its substantial impact on the quality of life of affected patients. Although firstly described in the literature nearly half a century ago, the diagnosis and treatment of this pathological condition remain major challenges for modern medicine. Consequently, diabetic gastroenteropathy is the subject of extensive scientific research aimed at improving the quality of life for all patients who develop this chronic complication. To achieve a standard in treatment, it is crucial to adequately educate physicians to ensure early recognition, emphasizing a multidisciplinary approach in providing healthcare for these patients.
