Abstract | Razni oblici kognitivnih poremećaja čine heterogenu skupinu u rasponu od blagih oštećenja do patoloških i starenjem izazvanih demencija. Najčešći među njima su vaskularna demencija (VaD) te Alzheimerova bolest (engl. Alzheimer's Disease, AD) i to kod osoba starijih od 65 godina. Razvoj demencije tipa AD može biti potaknut, a tipa VaD izravno uzrokovan lječivim somatskim stanjima, prvenstveno bolestima kardiovaskularnog sustava. Naime bolesti poput hipertenzije, hiperlipidemije, ateroskleroze, ali i dijabetesa dovode do oštećenja u cirkulaciji središnjeg živčanog sustava te uzrokuju krvarenja, ishemiju i ireverzibilna oštećenja tkiva (moždani udar) koja se, zahvate li ključne regije mozga, posljedično očituju u obliku demencija. Ovakva poveznica postavlja pitanje o farmakološkim učincima lijekova za kardiovaskularne poremećaje u primarnoj ili sekundarnoj prevenciji demencija. Skupina vrlo potentnih lijekova, koja se koristi za liječenje kardiovaskularnih bolesti, su statini odnosno inhibitori 3-metil-3-hidroksiglutaril-koenzim A reduktaze (HMG CoA –reduktaza) koji utječu na metabolizam kolesterola i snižavaju njegove vrijednosti u plazmi. Osim učinka na hiperlipidemiju, ova skupina lijekova sprječava razvoj ateroskleroze ili njeno daljnje napredovanje, a svojim protuupalnim učincima stabilizira plak i uzrokuje regeneraciju endotela. Još jedna bitna karakteristika statina je inhibicijsko djelovanje na acetilkolin-esterazu (AChE) koja potiče stvaranje plakova tipičnih za AD. Cilj ovog preglednog rada je utvrditi dosadašnja saznanja i istraživanja o ulozi statina u liječenju demencija. |

Parallel abstract (English) | Cognitive decline describes a heterogeneous condition with cognitive changes between mild impairment, normal ageing and dementia. The most common ones among population older than 65 years are Alzheimer's disease (AD) and vascular dementia (VaD). Cognitive impairment or dementia can be promoted or caused by treatable somatic factors, mainly cardiovascular disease. Hypertension, hyperlipidaemia, atherosclerosis and diabetes cause vascular damage which lead to bleeding, ischemia and irreversible tissue damage (stroke) which presents with various dementia symptoms. According to this link the question remains, can we use cardiovascular drugs to prevent or to treate demencia. Very potent cardiovascular drugs, statins (inhibitors of HMG-CoA reductase), play impotant role in metabolism of cholesterol by decreasing it's concentration in blood. This effect inhibits progression or occurrence of hypertension, atherosclerotic plaque and because of an anti-inflammatory effects statins stabilize the plaque and stimulates endothelial regeneration. Statins also inhibits acetylcholinesterase (AChE), molecule that induces production of plaque specific for AD. The aim of the study was to determine so far knowledge and researche papers about potentially preventive role of statins in vascular dementia. |
