Ovaj diplomski rad istražuje povijest i razvoj genske terapije, osnovne principe genetike, različite tehnike genske terapije, te kliničke primjere njene primjene u liječenju teških nasljednih bolesti kod djece. Povijesni pregled obuhvaća rane eksperimente s prijenosom gena i ključne tehnološke inovacije, uključujući razvoj virusnih vektora i tehnologija za uređivanje gena poput CRISPR- Cas9. Osnove genetike objašnjavaju strukturu i funkciju gena, te kako mutacije mogu dovesti do genetskih poremećaja. Glavni pristupi genskoj terapiji uključuju gensku nadomjesnu terapiju, gensku korekciju i antisense oligonukleotide, svaki sa svojim specifičnim mehanizmima djelovanja i kliničkim primjenama. Diplomski rad također detaljno razmatra genetske poremećaje u pedijatriji, uključujući spinalnu mišićnu atrofiju (SMA), Duchenneovu mišićnu distrofiju (DMD), hemofiliju A, srpastu anemiju, beta talasemiju i druge. Prikazani su klinički primjeri uspješne primjene genske terapije, ističući prednosti, kao što su dugoročno liječenje i poboljšanje kvalitete života, te izazove, uključujući imunološke reakcije i etička pitanja. Posebna pažnja posvećena je važnosti rane intervencije kod djece kako bi genska terapija bila što učinkovitija. Rana dijagnoza i početak terapije mogu spriječiti progresiju bolesti i značajno poboljšati ishode liječenja. U zaključku, genska terapija ima potencijal transformirati liječenje genetskih poremećaja u pedijatriji. Kontinuirano istraživanje, razvoj novih tehnologija i suradnja među znanstvenicima, kliničarima i regulatornim tijelima ključni su za ostvarivanje ovog potencijala. Genska terapija nudi nadu za liječenje nasljednih bolesti koje su do sada bile neizlječive i postavlja temelje za buduće inovacije u medicini koje će značajno poboljšati zdravlje i kvalitetu života djece širom svijeta.
Abstract (english) Gene therapy represents a revolutionary advancement in medicine, providing new possibilities for the treatment and prevention of genetic disorders, especially in the pediatric population. This thesis explores the history and development of gene therapy, fundamental principles of genetics, various gene therapy techniques, and clinical examples of its application in treating severe hereditary diseases in children. The historical overview covers early experiments with gene transfer and key technological innovations, including the development of viral vectors and gene editing technologies like CRISPR-Cas9. The fundamentals of genetics explain the structure and function of genes, and how mutations can lead to genetic disorders. The main approaches to gene therapy include gene replacement therapy, gene correction, and antisense oligonucleotides, each with its specific mechanisms of action and clinical applications. The thesis also thoroughly examines genetic disorders in pediatrics, including spinal muscular atrophy (SMA), Duchenne muscular dystrophy (DMD), hemophilia A, sickle cell anemia, beta-thalassemia, and others. Clinical examples of successful gene therapy applications are presented, highlighting benefits such as long-term treatment and improved quality of life, as well as challenges like immune responses and ethical issues. Special attention is given to the importance of early intervention in children to maximize the effectiveness of gene therapy. Early diagnosis and initiation of therapy can prevent disease progression and significantly improve treatment outcomes. In conclusion, gene therapy has the potential to transform the treatment of genetic disorders in pediatrics. Continued research, the development of new technologies, and collaboration among scientists, clinicians, and regulatory bodies are crucial to realizing this potential. Gene therapy offers hope for curing hereditary diseases that were previously untreatable and lays the foundation for future medical innovations that will significantly enhance the health and quality of life of children worldwide.
