Abstract

Esencijalni tremor (ET) najčešći je poremećaj pokreta (hiperkinezija) u ljudi. To je klinički sindrom najčešće akcijskog tremora gornjih ekstremiteta te rjeđe trupa i donjih udova , a često je prisutan i tremor glave te tremor glasnica bez prisutnih drugih neuroloških znakova. Može početi u bilo koje doba života (djetinjstvo, adolescencija, odraslo ili starije doba). Karakterističan znak ET-a je dominantno kinetički tremor. ET je u oko polovice oboljelih nasljedna autosomno dominantna (AD) bolest, no patofiziološka podloga ET-a je još uvijek nepoznata. Iako u većini slučajeva nije jako izražen, u nekih osoba ET može izazvati socijalnu neugodu. Može uzrokovati poteškoće u pisanju i ostalim finim radnjama (pijenje vode iz čaše, hranjenje, oblačenje i govorenje), a u nekim slučajevima pacijent je u potpunosti onesposobljen za svakodnevne aktivnosti. Poboljšanje tremora nakon konzumiranja alkohola gotovo je patognomoničan znak u nekih oblika ET-a. Krivo postavljena dijagnoza ET-a nije rijetka, jer kliničari često previde ostale neurološke simptome, ali i zato jer je akcijski tremor šaka izazvan mnogim drugim stanjima kao što su: Parkinsonova bolest (PB), distonija – distonički tremor, lijekovima induciran tremor i dr. Metode liječenja ET-a uključuju terapiju lijekovima te neurokirurške metode kao što je duboka moždana stimulacija (engl. deep brain stimulation - DBS). Obzirom na nepoznat uzrok i učestalost javljanja, ET predstavlja izazov za istraživače i kliničare.

Abstract (english)

Essential tremor (ET) is the most common movement disorder (hyperkinesia) in humans. It is a clinical syndrome of action tremor in the upper limbs and less commonly trunk, and lower limbs, but head tremor and vocal chords tremor are frequently present. Other neurological signs should be absent. It can start in every period of life (childhood, adolescence, adulthood or older age). The characteristic sign of ET is predominantly kinetic tremor. In about half of patients the disease is hereditary autosomal dominant, however the pathophysiologic basis of ET is still unknown. In most cases this tremor is harmless and the main problems are cosmetic and the social discomfort it causes. It can cause difficulties in writing and other fine actions (drinking water from glass, feeding, clothing, and speaking) and in some cases a patient is completely unable to do everyday activities. Improvement of tremor after alcohol consumption is almost a pathognomonic sign of some types of ET. Misdiagnosis of ET is not rare because clinicians frequently overlook other neurologic signs and because action tremor in the hands is caused by many other conditions such as dystonia- dystonic tremor, Parkinson disease (PD) and drug-induced tremor. Methods of treating ET are pharmacologic treatment and neurosurgical methods such as deep brain stimulation. ET presents the challenge for researchers and clinicians because of unknown aetiologies of this syndrome.
