Učinak hibridnog sustava zatvorene petlje na kontrolu glikemije u bolesnika sa šećernom bolesti tipa 1

Abstract

Šećerna bolest tipa 1 je kronična, autoimuna bolest koja proizlazi iz uništenja ꞵ-stanica gušterače, što rezultira simptomima hiperglikemije. Liječenje se provodi nadomjesnom inzulinskom terapijom putem inzulinskih penova ili inzulinskih pumpi. U posljednje vrijeme, sustavi zatvorene petlje su intenzivno istraživani, a na tržištu je dostupno nekoliko hibridnih sustava zatvorene petlje. Ovi sustavi pokazuju značajno poboljšanje regulacije glikemije, što može dovesti do smanjenja kroničnih komplikacija šećerne bolesti tipa 1. Cilj ovog istraživanja je bio proučiti, analizirati i evaluirati učinak hibridnog sustava zatvorene petlje na parametre regulacije glikemije. Istraživanje je provedeno na 26 ispitanika (12 muškaraca), medijana dobi 32,0 godine uz IQR 14,8 godina, s dijagnozom šećerne bolesti tipa 1, a koji su koristili hibridni sustav zatvorene petlje u svom liječenju tijekom 6 mjeseci. Analizom dobivenih podataka utvrđeno je poboljšanje regulacije glikemije, koje se očitovalo povećanjem postotka vremena provedenog unutar ciljnog raspona (TIR) s 58,77 na 77,73%, smanjenjem indeksa upravljanja glukozom (GMI) sa 7,25%% na 6,72% te smanjenjem prosječne vrijednosti glukoze iz senzora (AG) s 9,07 mmol/L na 7,89 mmol/L. Na temelju rezultata hibridni sustavi zatvorene petlje pozitivno utječu na regulaciju glikemije kod osoba sa šećernom bolešću tipa 1.

Abstract (english)

Type 1 diabetes is a chronic, autoimmune disease resulting from the destruction of pancreatic β-cells, leading to symptoms of hyperglycemia. Treatment involves insulin replacement therapy via insulin pens or insulin pumps. Recently, closed-loop systems have been extensively researched, and several hybrid closed-loop systems are commercially available. These systems demonstrate significant improvements in glycemic regulation, which may lead to reduced chronic complications of type 1 diabetes. The aim of this study was to examine, analyze, and evaluate the effect of a hybrid closed-loop system on glucose management parameters. The study was conducted on 26 participants (12 men), with a median age of 32.0 years and an interquartile range (IQR) of 14.8 years, all diagnosed with type 1 diabetes, who used a hybrid closed-loop system in their treatment for 6 months. Analysis of the obtained data revealed an improvement in glycemic control, evidenced by an increase in the percentage of time spent within the target range (TIR) from 58.77% to 77.73%, a decrease in the glucose management indicator (GMI) from 7.25% to 6.72%, and a reduction in the average glucose value from the sensor (AG) from 9.07 mmol/L to 7.89 mmol/L. Based on the results, hybrid closed-loop systems positively impact glycemic control in individuals with type 1 diabetes.
