Abstract

Psihijatrijske bolesti važan su javnozdravstveni izazov kako u Hrvatskoj tako i u svijetu. Pacijenti koji se suočavaju s tim poremećajima često imaju i somatske bolesti kao komorbiditete, što dodatno komplicira njihovo liječenje. Zbog toga nije rijetka pojava da ti pacijenti istovremeno koriste različite lijekove za tretiranje svojih psihičkih i fizičkih tegoba. Još 1960-ih godina prepoznato je da lijekovi mogu imati međusobne interakcije koje bitno utječu na terapijski ishod. Razumijevanje farmakokinetičkih i farmakodinamičkih aspekata tih interakcija od ključne je važnosti za pružanje optimalne skrbi pacijentima. Farmakokinetika se bavi procesima apsorpcije, distribucije, metabolizma i eliminacije lijekova u organizmu, dok farmakodinamika proučava biološke učinke lijekova i mehanizme njihova djelovanja na receptore, a kombinacija tih disciplina omogućuje razumijevanje kako lijekovi međusobno djeluju i kako to može utjecati na terapijski učinak. Interakcije psihofarmaka međusobno, ali i s drugim lijekovima te s hranom su od iznimne važnosti jer mogu značajno utjecati na efikasnost i sigurnost terapije. Integrirani pristup koji uključuje razumijevanje farmakokinetičkih i farmakodinamičkih navedenih interakcija omogućuje primjenu individualizirane terapije, što minimizira rizik od štetnih interakcija istovremeno osiguravajući optimalnu sigurnost i efikasnost liječenja pacijenata sa psihičkim poremećajima i somatskim komorbiditetima. Takav pristup ne samo da poboljšava terapijske ishode, nego i znatno pridonosi ukupnoj kvaliteti života pacijenata smanjujući rizik od nuspojava i povećavajući učinkovitost liječenja.

Abstract (english)

Psychiatric illnesses represent a significant public health challenge both in Croatia and worldwide. Patients dealing with these disorders often have somatic diseases as comorbidities, which further complicates their treatment. Consequently, it is not uncommon for these patients to simultaneously use different medications to treat their psychological and physical ailments. Since the 1960s, it has been recognized that different drugs can have interactions that significantly impact therapeutic outcomes. Understanding these interactions' pharmacokinetic and pharmacodynamic aspects is crucial for optimal patient care. Pharmacokinetics deals with the processes of absorption, distribution, metabolism, and elimination of drugs in the body, while pharmacodynamics studies the biological effects of drugs and their mechanisms on cell receptors. The combination of these disciplines enables an understanding of how drugs interact with each other and how this can affect the therapeutic outcomes. Interactions between psychopharmaceuticals themselves, but also with other medications and food, are of utmost importance as they can significantly impact the efficacy and safety of the therapy.An integrated approach that includes understanding pharmacokinetic and pharmacodynamic of aforementioned interactions allows for the application of individualized therapy, which minimizes the risk of harmful effects while ensuring the optimal safety and efficacy of treatment for patients with psychiatric disorders and somatic comorbidities. Such an approach improves therapeutic outcomes and significantly contributes to patients' overall quality of life, reducing the risk of side effects and increasing treatment efficiency.
