Abstract

Prijelomi femura ubrajaju se u češće prijelome u dječjoj dobi. Cilj ovog istraživanja je utvrditi najčešće mehanizme i mjesta nastanka prijeloma femura u djece. U ovu retrospektivnu studiju uključeno je 103 djece s ukupno 106 prijeloma femura u dobi do 18. godine života, liječene u Kliničkom bolničkom centru Zagreb i Klinici za dječje bolesti Zagreb. Mehanizmi nastanka prijeloma te mjesta na kojima su se prijelomi dogodili kodirani su radi jednostavnosti statističke analize podataka šiframa Međunarodne klasifikacije bolesti i srodnih zdravstvenih problema. Od 106 prijeloma femura 67 % dogodio se u dječaka, a 33 % u djevojčica. Najveći broj ozlijeđene djece bio je u najmlađoj dobnoj skupini, od 0. do 4. godine života. Medijan dobi pri nastanku prijeloma bio je 5 godina. U području dijafize bilo je 62 % svih prijeloma. U proksimalnom dijelu femura 25 %, a u distalnom 13 %. Kod kuće je nastalo 38 %, na ulici ili cesti 36 %, na rekreacijskom mjestu 21 % te 3 % prijeloma nastali su u školi ili vrtiću. Prijelomi femura najčešće su uzrokovani padovima, nezgodama s motornim vozilima te sudaranjem ili izravnim udarcem. Do pridruženih je ozljeda došlo u 33 % slučajeva prijeloma femura. One su u 80 % slučajeva uzrokovane nezgodama s motornim vozilima. Rezultati ovog istraživanja pokazuju da su prijelomi femura najučestaliji u najmlađim dobnim skupinama, a najčešće nastaju kao posljedica nezgoda kod kuće, na ulici ili cesti te na rekreacijskim mjestima. Prijelomi su najčešće posljedica padova i nezgoda s motornim vozilima. Na temelju dobivenih rezultata možemo zaključiti da bi se preventivnim intervencijama za povećanje sigurnosti djece na rekreacijskim mjestima i u cestovnom prometu mogla smanjiti učestalost prijeloma femura u djece.

Abstract (english)

Femoral fractures are among the most common fractures in children. The aim of this research was to determine the most frequent mechanisms and femoral fracture sites in children. This retrospective study included 103 children up to 18 years in age, with 106 femoral fractures, treated at the University Hospital Centre of Medicine Rebro and Children's Hospital Zagreb. The mechanisms and sites of the fractures are encrypted by ICD-10 classification for the simplicity of statistical analysis. Out of 106 femoral fractures, 67 % occurred in boys and 33 % in girls. The majority of injured children were in the youngest age group, from 0 to 4 years of age. Median of age at the time of fracture was 5 years. 62 % of all fractures were localised at the diaphysis area, 25 % in the distal and 13 % in the proximal part of the femur. 38 % of all fracture cases happened at home, 36 % in the street or road, 21 % at recreational places and 3 % at school or in kindergarten. Femoral fractures were most frequently caused by falls, motor vehicle accidents and collision or direct hit. Associated injuries occurred in 33 % of femoral fracture cases and 80 % of those cases were caused by motor vehicle accidents. The results of this study show that femoral fractures most commonly occur in the youngest age groups and they are most frequently caused by accidents at home, in the street or road and at recreational places. Femoral fractures are most commonly the result of falls and motor vehicle accidents. Based on the obtained results we can conclude that some preventive interventions for the increase of children's safety at recreational places and in road traffic could decrease the frequency of femoral fractures in children.
